摘要
目的探讨血清中性粒细胞与淋巴细胞比值(NLR)和降钙素原(PCT)预测川崎病儿童伴肺部感染的应用价值。方法本研究为病例对照研究。采用便利抽样法,选取2019年3月至2021年3月辽阳市中心医院诊断川崎病儿童124例,经影像学检查诊断肺部感染例48例(观察组)和无肺部感染76例(对照组)。比较2组患者治疗前后血清NLR和PCT水平。结果治疗前观察组血清NLR和PCT水平均明显高于对照组(P值均<0.05),治疗后2组血清NLR和PCT水平均较治疗前降低(P值均<0.05),但治疗后2组血清NLR和PCT水平比较,差异均无统计学意义(P值均>0.05)。二元logistic回归分析显示,治疗前NLR和PCT水平是川崎病伴肺部感染的危险因素(P值均<0.05)。受试者工作特征曲线显示治疗前血清NLR和PCT水平及两项联合预测川崎病伴肺部感染的曲线下面积分别为0.85、0.87和0.90(P值均<0.05)。结论川崎病儿童伴肺部感染血清NLR和PCT水平升高,对预测肺部感染有较好的价值。该研究创新点是为临床早期鉴别诊断川崎病和肺部感染,以及识别川崎病伴肺部感染提供了重要敏感性生化标志物,具有较好的临床操作价值。
Objective To explore the values of serum neutrophil to lymphocyte ratio(NLR)and procalcitonin(PCT)for predicting lung infection in children of Kawasaki disease.Methods This was a case-control study.A total of 124 children with Kawasaki disease admitted to Liaoyang Central Hospital from March 2019 to March 2021 were selected by convenience sampling method,and 48 children(observation group)and 76 children(control group)were diagnosed with pulmonary infection by imaging examination.Serum NLR and PCT levels before and after treatment were compared between the two groups.Results The serum NLR and PCT levels before treatment in the observation group were significantly higher than those in the control group(both P<0.05),and the levels after treatment in the two group were lower than those before the treatment(all P<0.05).There showed no statistical difference in the two groups after treatment(both P<0.05).Binary logistic regression analysis showed that the levels of NLR and PCT before treatment were the risk factors of Kawasaki disease with pulmonary infection(both P<0.05).The subjects'operating characteristic curves showed that the areas under the curves of serum NLR and PCT levels before treatment alone or combined in predicting of kawasaki disease with pulmonary infection were 0.85,0.87 and 0.90,respectively(all P<0.05).Conclusions Serum NLR and PCT levels in children of Kawasaki disease with pulmonary infection are higher,which has high value in predicting pulmonary infection.The innovation is to provide important sensitive biochemical markers for early clinical differential diagnosis of Kawasaki disease and pulmonary infection,as well as for the identification of Kawasaki disease with pulmonary infection.
