某三甲医院血药浓度监测情况回顾性分析 被引量:1

摘要 目的为促进临床合理用药,积极发挥血药浓度监测的作用,对2020年度血药浓度监测情况进行回顾性分析。方法对本院2020年开展的药物浓度监测情况进行回顾性分析,包括患者信息、监测例次、频次,监测结果、住院患者监测情况和监测科室分布等情况。结果2020年本院共监测了11种药物,共计1610例次,监测例次排名前三位的药物分别是丙戊酸(26.46%)、他克莫司(22.11%)和环孢素A(19.19%),监测例次排名后三位的药物分别是苯巴比妥(0.87%)、茶碱(0.37%)和霉酚酸(0.19%)。监测频次排名前三位的药物分别是他克莫司、环孢素A和丙戊酸,而地高辛、霉酚酸、茶碱大多数仅监测1次。所监测的血药浓度结果在有效浓度范围内的比例达55.56%。住院患者监测比例排名前三位的药物分别是霉酚酸、苯妥英钠和他克莫司,监测比例分别为100.00%、40.00%、27.00%,地高辛监测比例为20.55%,而其他药物监测比例均低于20%。血药浓度监测涉及34个临床科室和简易门诊,其中血液内科、肾脏内科及神经内科监测例次位居前三位。结论血药浓度监测已在本院普及开展,少数临床医师已经认识血药浓度监测的重要性,但仍有部分药物品种的血药浓度监测易被临床医师忽视。临床药师仍需积极宣传血药浓度监测的意义,提高临床医师的合理监测意识,从而保证患者用药安全、有效。 Objective In order to promote the rational use of drugs,and play a positive role of blood concentration monitoring,a retrospective analysis of the blood concentration monitoring in 2020 in our hospital was conducted.Methods A retrospective analysis was conducted on drug concentration monitoring in our hospital from January to December 2020,including patient information,monitoring frequency,monitoring ratio,monitoring results,of hospitalized patients,department distribution,etc.Results From January to December 2020,totally 1610 cases of 11 drugs were monitored in our hospital.The top three drugs in monitoring cases were valproic acid(26.46%),tacrolimus(22.11%)and cyclosporine(19.19%).The bottom three drugs in monitoring cases were phenobarbital(0.87%),theophylline(0.37%)and mycophenolic acid(0.19%).The top three monitoring frequency of drugs were tacrolimus,cyclosporine,and valproic acid,while most of digoxin,theophylline,and mycophenolic acid were monitored only once.The proportion of monitoring results of blood concentration those within the effective blood concentration monitoring range reached 55.56%.The top three inpatient monitoring rates were mycophenolic acid,phenytoin,and digoxin.The monitoring rates were 100.00%,40.00%,and 20.55%,while other drug monitoring ratios were less than 20%.The blood concentration monitoring in our hospital involves 34 clinical departments and simple outpatient clinics,among which the top three monitoring times are the department of hematology,nephrology and neurology.Conclusions Drug blood concentration monitoring has been widely carried out in our hospital.A few clinicians have realized the importance of blood concentration monitoring,but the blood concentration monitoring of some drugs has not yet received the attention of clinicians.Clinical pharmacists still need to strengthen cooperation with clinicians,actively publicize the significance of blood drug concentration monitoring,improve clinicians’reasonable monitoring awareness,and ensure that patients’clinical medication is safe and effective.
