摘要
目的:分析乙酰半胱氨酸雾化吸入联合振动机械排痰治疗脑外伤后坠积性肺炎的效果。方法:选取2020年1月至2021年1月河南科技大学第二附属医院收治的82例脑外伤后坠积性肺炎患者,根据治疗方法不同将其分为对照组40例,观察组42例。对照组使用振动机械排痰治疗,观察组在振动机械排痰的基础上给予乙酰半胱氨酸雾化吸入治疗。比较两组患者治疗总有效率、并发症发生情况、治疗前后炎症因子水平、生活质量。结果:观察组总有效率为92.86%,高于对照组75.00%,且观察组治疗后白细胞介素-1β(IL-1β)、肿瘤坏死因子-α(TNF-α)水平低于对照组,差异具有统计学意义(P<0.05);观察组治疗后躯体功能、心理功能、社会功能评分均高于对照组,差异具有统计学意义(P<0.05);观察组并发症发生率为7.14%,高于对照组5.00%,但差异无统计学意义(P>0.05)。结论:乙酰半胱氨酸雾化吸入联合振动机械排痰治疗脑外伤后坠积性肺炎患者,能减轻患者炎症因子水平,提高其生活质量,且并发症发生率较低。
Objective To analyze the effect of atomization inhalation of acetylcysteine combined with vibration mechanical expectoration in the treatment of post-traumatic hypostatic pneumonia.Methods A total of 82 patients with pneumonia after traumatic brain injury admitted to the Second Affiliated Hospital of Henan University of Science and Technology from January 2020 to January 2021 were divided into a control group with 40 cases and an observation group with 42 cases according to different treatment methods.The control group was treated with vibrating mechanical expectoration,and the observation group was treated with aerosol inhalation of acetylcysteine on the basis of vibrating mechanical expectoration.The total effective rate,incidence of complications,levels of inflammatory factors before and after treatment and quality of life were compared between the two groups.Results The total effective rate in the observation group was 92.86%,which was higher than 75.00% in the control group,and the levels of IL-1β and TNF-α in the observation group were lower than those in the control group(P<0.05).After treatment,the scores of physical function,psychological function and social function in the observation group were higher than those in the control group,the differences were statistically significant(P<0.05).The incidence of complications in the observation group was 7.14%,which was higher than 5.00% in the control group,and there was no statistical significance between the two groups(P>0.05).Conclusion Acetylcysteine atomization inhalation combined with vibration mechanical expectoration is effective in the treatment of hypostatic pneumonia after brain trauma,which can significantly reduce the inflammatory level,and improve quality of life,with a low incidence of complications.
