Effect of refined intervention on pulmonary function and activity tolerance in patients with lung cancer after surgery

摘要 目的 探讨精细化干预对肺癌术后患者肺功能、活动耐力的影响。方法 将106例行肺癌手术的患者根据干预方式不同分为对照组(n=53)和观察组(n=53)。对照组患者采取常规干预,观察组患者在对照组的基础上采取精细化干预。比较两组患者手术相关指标、肺功能、活动耐力以及并发症发生情况。结果 观察组患者下床时间、术后排气时间及术后住院时间均明显短于对照组,差异均有统计学意义(P﹤0.01)。干预后,两组患者用力肺活量(FVC)、第1秒用力呼气容积(FEV)、FEV/FVC均较本组干预前升高,且观察组患者FVC、FEV、FEV/FVC均高于对照组,差异均有统计学意义(P﹤0.05)。干预后,两组患者6分钟步行试验距离(6MWTD)及改良版英国医学研究委员会呼吸困难量表(MMRC)评分均较本组干预前降低,但观察组患者6MWTD高于对照组,MMRC评分低于对照组,差异均有统计学意义(P﹤0.05)。干预后,观察组患者并发症总发生率低于对照组患者,差异有统计学意义(P﹤0.05)。结论 精细化干预能够提高肺癌术后患者的肺功能和运动耐力,缩短患者的下床时间、排气时间及住院时间,降低患者并发症总发生率。 Objective To investigate the effect of refined intervention on pulmonary function and activity tolerance in patients with lung cancer after surgery. Method A total of 106 patients with lung cancer surgery were divided into control group(n=53) and observation group(n=53) according to different intervention methods. The patients in the control group received the routine intervention, while the patients in the observation group received refined intervention based on the routine regimen. The operation-related indexes, pulmonary function, activity tolerance, and the incidence of complications were compared between the two groups. Result The time to get out of bed, postoperative exhaust time,and postoperative hospital stay in the observation group were significantly shorter than those in the control group, and the differences were statistically significant(P<0.01). After the intervention, the forced vital capacity(FVC), forced expiratory volume in one second(FEV), and FEV/FVC of the two groups were increased compared with those before the intervention, and FVC, FEV, and FEV/FVC of the observation group were higher than those of the control group, the differences were statistically significant(P<0.05). After the intervention, the six minutes walk test distance(6MWTD) and modified Medical Research Council dyspnea scale(MMRC) scores of the two groups were lower than those before the intervention, while the 6MWTD of the observation group was higher than that of the control group, and the MMRC score was lower than that of the control group, the differences were statistically significant(P<0.05). After the intervention, the total incidence of complications in the observation group was lower than that in the control group, and the difference was statistically significant(P<0.05). Conclusion Refined intervention could improve the lung function and activity tolerance of patients after lung cancer surgery, shorten their getting out of bed time, exhaust time, and hospitalization time, and reduce the total incidence of complications.
