摘要
目的探讨“富集法”全自动粪便分析仪在粪便分析中的检测性能和临床应用价值。方法配备含不同浓度蛔虫卵、WBC的模拟粪便悬液,确定最低检出限,配制极低浓度虫卵、WBC的模拟样本,确认极低浓度样本检出率,根据富集体积,将转膜后的有形成分洗脱,确认有形成分回收率,仪器法、人工法共同检测临床样本831例,并对两种分析方法检测结果进行统计分析。结果“富集法”全自动粪便分析仪虫卵最低检出限为0.078个/μL,WBC最低检出限为0.313个/μL,极低浓度标本检出率为100%,有形成分回收率为88.11%,检测虫卵、WBC、真菌、脂肪滴灵敏度分别为95.15%,86.36%,75.00%,51.52%,特异度分别为98.21%,98.60%,99.26%,99.75%,符合率分别为97.83%,97.95%,98.80%,97.83%,Kappa值分别为0.903,0.806,0.700,0.644。两种方法检测虫卵、WBC、真菌结果差异均无统计学意义(P>0.05),检测脂肪球结果差异有统计学意义(P<0.05)。结论“富集法”全自动粪便分析仪检测虫卵、细胞有较低检出限和较高有形成分回收率,与人工法比较,仪器法检测虫卵、WBC、真菌有较高的灵敏度、特异性、符合率。
Objective To explore the detection performance and clinical application value of the"enrichment method"automatic stool analyzer in stool detection.Methods Equipped with simulated fecal suspension containing different concentrations of roundworm eggs and WBC to determine the minimum detection limit,configured a simulated sample of extremely low concentration of fungi,WBC,and confirmed the detection rate of extremely low concentration samples.according to the enrichment volume,eluted the formed components after transfer,and the recovery rate of formed elements was confirmed,the instrument method and manual method jointly detected clinical sample cases,and analyzed the detection results of the two analytical methods.Results The minimum detection limit of eggs,WBC,fungi and fat drops was 0.078/μL,0.313/μL.The detection rate of very low concentration samples was 100%,and the recovery rate of tangible components was 88.11%.The sensitivity of detecting eggs,WBC,fungi and fat droplets were 95.15%,86.36%,75.00%and 51.52%,respectively.The specificity was 98.21%,98.60%,99.26%and 99.75%,respectively.The coincidence rate was 97.83%,97.95%,98.80%,97.83%,respectively.Kappa values were 0.903,0.806,0.700 and 0.644,respectively.There was no significant difference in the results of egg,WBC and fungi between the two methods(P>0.05),but there was significant difference in fat drop detection(P<0.05).Conclusion The"enrichment method"automatic stool analyzer has a lower detection limit and a higher recovery rate of formed components for the detection of eggs and cells,compared with the manual method,the instrument method has higher sensitivity for the detection of eggs,WBC,and fungi.
