摘要
目的观察安宫牛黄丸治疗脓毒症相关性脑病(SAE)患者的临床疗效。方法将重症监护室(ICU)SAE患者90例采用随机数字表法分对照组与治疗组各45例。对照组患者予以西医常规治疗,治疗组患者在对照组基础上加安宫牛黄丸,温水溶开后鼻胃管注入,两组疗程均为5 d。比较两组患者治疗结束后临床疗效;检测两组患者治疗前后血清神经元特异性烯醇化酶(NSE)、S100钙结合蛋白(S-100β)、C型利钠肽及其氨基末端前肽(NT-proCNP)水平及血浆降钙素原(PCT)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平。结果治疗后,两组患者中医证候评分均下降,治疗组下降更显著(P<0.01);两组患者格拉斯哥昏迷量表(GCS)评分均升高,治疗组升高更显著(P<0.01);两组患者血清NSE、S-100β、NT-proCNP、PCT、IL-6、TNF-α水平均下降,治疗组下降更显著(P<0.05)。结论安宫牛黄丸可显著减轻SAE患者炎症反应,下调血清NSE、S-100β、NT-proCNP水平,保护SAE患者神经功能,临床应用效果显著。
Objective:To observe the clinical efficacy of Angong Niuhuang Pills for patients with sepsis-associ⁃ated encephalopathy(SAE).Methods:Ninety patients with SAE in intensive care unit(ICU)were divided into the control group and treatment group by random number table method,45 cases in each group.The control group received conventional western medicine treatment,while the treatment group were added with Angong Niuhuang Pills on the basis of the control group,dissolved in warm water,and then injected into the nasogastric tube.The course of treatment in both groups was 5 days.The clinical efficacy of the two groups after treatment was com⁃pared;serum NSE,S-100β,NT-proCNP levels and plasma PCT,IL-6,TNF-αlevels before and after treatment were detected in the two groups.Results:After treatment,the TCM syndrome scores of the two groups were de⁃creased,and the decrease was more significant in the treatment group(P<0.01).Serum leuels of NSE,S-100β,NT-proCNP,PCT,IL-6,and TNF-αwere decreased,and the decrease was more significant in the treatment group(P<0.05).Conclusion:Angong Niuhuang Pills can significantly reduce the inflammatory response in patients with SAE,down-regulate the levels of serum NSE,S-100β,NT-proCNP,and protect the neurological function of patients with SAE,and the clinical application effect is significant.
