高渗盐水雾化吸入联合气道廓清技术对急性加重期慢性阻塞性肺疾病患者缺氧症状及排痰的疗效分析 被引量:1

摘要 目的 探讨高渗盐水雾化吸入联合气道廓清技术治疗急性加重期慢性阻塞性肺疾病(COPD)患者的效果。方法 选取2019年1月至2020年1月本院急性加重期COPD患者76例作为研究对象,根据治疗方式不同分为对照组和观察组,各38例。对照组接受高渗盐水雾化吸入治疗,观察组在对照组基础上联合气道廓清技术展开治疗,比较两组排痰难度、呼吸困难指数、动脉血气指标及肺功能指标。结果 治疗1周后,两组排痰难度评分均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);治疗1周后,两组呼吸困难指数均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);观察组动脉血氧分压(PaO_(2))和动脉血二氧化碳分压(PaCO_(2))水平低于对照组,差异有统计学意义(P<0.05);观察组第1秒用力呼吸容积(FEV_(1))、用力肺活量(FVC)高于对照组,差异有统计学意义(P<0.05)。结论 高渗盐水雾化吸入联合气道廓清技术治疗急性加重期COPD效果显著,能改善患者动脉血气指标及肺功能,促进排痰,值得临床推广使用。 Objective To investigate the effect of hypertonic saline aerosol inhalation combined with airway clearance in the treatment of patients with acute exacerbation of chronic obstructive pulmonary disease(COPD). Methods A total of 76 patients with acute exacerbation COPD in our hospital from January 2019 to January 2020 were selected as the research subjects, and they were divided into control group and observation group according to different treatment methods, with 38 cases in each group. The control group received hypertonic saline aerosol inhalation therapy,the observation group was treated with airway clearance technology on the basis of the control group, and the sputum expectoration difficulty, dyspnea index, arterial blood gas index and pulmonary function index were compared between the two groups. Results After 1 week of treatment, the difficulty of expectoration scores in both groups were lower than before treatment, and the observation group was lower than that in the control group, and the difference was statistically significant(P<0.05). After 1 week of treatment, the dyspnea index in the two groups were lower than before treatment,and the observation group was lower than that in the control group, and the difference was statistically significant(P<0.05);the levels of arterial partial pressure of oxygen(PaO_(2)) and arterial partial pressure of carbon dioxide(PaCO_(2)) in the observation group were lower than those in the control group, and the difference was statistically significant(P<0.05). The forced expiratory volume in one second(FEV_(1)) and forced vital capacity(FVC)in the observation group were higher than those in the control group, and the difference was statistically significant(P<0.05). Conclusion The use of hypertonic saline inhalation combined with airway clearance technology in patients with acute exacerbation of COPD has a significant effect, can improve the arterial blood gas index and lung function, and promote sputum expectoration, which is worthy of clinical promotion.
