摘要
目的 探究白细胞介素-6(IL-6)/信号转导和转录激活因子3(STAT3)信号通路对B族链球菌(GBS)感染孕妇早产的预测价值。方法 选择2019年2月-2021年2月绍兴市中心医院医共体总院就诊的GBS感染孕妇112例,根据GBS感染患者的妊娠结局,将其分为早产组32例和足月分娩组80例,比较两组孕妇一般资料、IL-6水平、STAT3蛋白[磷酸化细胞信号传导与转录活化因子3(p-STAT3)、磷酸化蛋白激酶B(p-Akt)、核因子(NF)-κB亚基p-65]表达水平、阴道病原菌及孕妇GBS血清分型,探究IL-6/STAT3信号通路在GBS感染早产中的作用。结果 早产组细菌性阴道炎(BV)、外阴阴道假丝酵母菌病(VVC)发生率均高于足月分娩组(P<0.05),两组滴虫发生率比较,无统计学差异;早产组IL-6水平高于足月分娩组(P<0.05);蛋白质印迹法蛋白表达显示,早产组p-STAT3、p-Akt、p-65灰度值均高于足月分娩组(P<0.05);预测GBS感染孕妇早产时,IL-6、p-STAT3、p-Akt、p-65的受试者工作特征曲线(ROC)曲线下面积分别为0.803、0.868、0.842、0.937;早产组孕妇中GBS基因型为:Ⅲ型24例为主占比75.00%,足月分娩组孕妇的基因型Ⅲ型42例占比52.50%。结论 GBS感染后可能通过调控IL-6/STAT3信号通路及影响微生态平衡而引发孕妇早产,而早产组孕妇中GBS基因型以Ⅲ型为主,因而在临床预防及治疗中还需关注Ⅲ型孕妇,避免造成不良妊娠结局。
OBJECTIVE To explore the value of interleukin-6/signal transducer and activate transcription 3(IL-6/STAT3) signaling pathways in prediction of premature delivery of pregnant women with group B Streptococcus(GBS) infection. METHODS A total of 112 pregnant women with GBS infection who were treated in General Hospital of Shaoxing Central Hospital from Feb 2019 to Feb 2021 were enrolled in the study and were divided into the premature delivery group with 32 cases and the full-term delivery group with 80 cases according to the pregnancy outcomes of the patients with GBS infection. The clinical data, IL-6 level and expression levels of STAT3 proteins [phosphorylated cell signal transduction and activator of transcription factor 3(p-STAT3), phosphorylated protein kinase B(p-Akt), nuclear factor(NF)-κB subunit p-65] were compared between the two groups of pregnant women, vaginal pathogens and serotypes of GBS were analyzed, and the effect of IL-6/STAT3 signaling pathways on premature delivery of the pregnant women with GBS infection was observed. RESULTS The incidence rates of bacterial vaginitis(BV) and vulvovaginal candidiasis(VVC) of the premature delivery group were significantly higher than those of the full-term delivery group(P<0.05), there was no significant difference in the incidence of trichomonas between the two groups. The IL-6 level of the premature delivery group was significantly higher than that of the full-term delivery group(P<0.05). The results of Western blot showed that the gray values of p-STAT3, p-Akt and p-65 were significantly higher in the premature delivery group than in the full-term delivery group(P<0.05). The areas under receiver operating characteristic(ROC) curves of IL-6, p-STAT3, p-Akt and p-65 were respectively 0.803, 0.868, 0.842 and 0.937 in prediction of premature delivery of the pregnant women with GBS infection. In the premature delivery group, the pregnant women with GBS genotype Ⅲ accounted for 75.00%(24 cases);the pregnant women with genotype Ⅲ accounted for 52.50%(42 cases) in the full-term delivery group. CONCLUSION The GBS infection may trigger the premature delivery of the pregnant women by regulating IL-6/STAT3 signaling pathway and affecting microecological balance, the pregnant women with GBS genotype III are dominant in the premature delivery group, therefore, it is necessary to pay close attention to the pregnant women with genotype III during the clinical prevention and treatment so as to prevent adverse pregnancy outcomes.
