摘要
目的:探讨健脾疏肝降逆法联合奥美拉唑治疗肝胃郁热型反流性食管炎(RE)的有效性及安全性。方法:将当阳市中医医院2019年2月-2021年8月收治的81例肝胃郁热型RE患者按照随机数字表法分为对照组40例和观察组41例,对照组给予奥美拉唑治疗,观察组给予健脾疏肝降逆法联合奥美拉唑治疗,观察两组临床疗效、胃动素及胃泌素水平、食管黏膜损伤度、中医症候积分及不良反应发生率。结果:观察组治疗后总有效率(90.24%)高于对照组(72.50%)(P<0.05)。治疗前,两组胃动素、胃泌素对比,差异均无统计学意义(P>0.05);治疗4周后,两组胃动素、胃泌素水平较治疗前均升高,且观察组均高于对照组(P<0.05)。观察组治疗后食管黏膜损伤度低于对照组(P<0.05)。治疗前,两组中医症候积分对比,差异均无统计学意义(P>0.05);治疗4周后,两组中医症候积分较治疗前均降低,且观察组均低于对照组(P<0.05)。观察组不良反应发生率(7.32%)低于对照组(25.00%)(P<0.05)。结论:健脾疏肝降逆法联合奥美拉唑治疗肝胃郁热型RE患者可改善胃肠激素水平和食管黏膜损伤度,临床效果更加,且安全性好。
Objective:To investigate the efficacy and safety of Jianpi Shugan Jiangni method combined with Omeprazole in the treatment of reflux esophagitis(RE)with stagnant heat in liver and stomach.Method:A total of 81 patients with RE patients with stagnant heat in liver and stomach who treated in Dangyang Hospital of Traditional Chinese Medicine from February 2019 to August 2021 were divided into the control group(n=40)and the observation group(n=41)according to the random number table method.The control group was given Omeprazole treatment,the observation group was treated with Jianpi Shugan Jiangni method combined with Omeprazole,and the clinical efficacy,motilin and gastrin levels,esophageal mucosal damage,TCM syndrome scores and incidence of adverse reactions were observed in two groups.Result:The total effective rate(90.24%)of the observation group after treatment was higher than that(72.50%)of the control group(P<0.05).Before treatment,there were no significant differences in motilin and gastrin between two groups(P>0.05);after 4 weeks of treatment,the motilin and gastrin levels in two groups were increased compared with those before treatment,and the observation group were higher than those of the control group(P<0.05).The esophageal mucosal damage in the observation group after treatment was lower than that in the control group(P<0.05).Before treatment,there were no significant differences in TCM syndrome scores between two groups(P>0.05);after 4 weeks of treatment,the TCM syndrome scores in two groups were decreased compared with those before treatment,and the observation group were lower than those of the control group(P<0.05).The incidence rate of adverse reactions(7.32%)in the observation group was lower than that(25.00%)of the control group(P<0.05).Conclusion:Jianpi Shugan Jiangni method combined with Omeprazole can improve the level of gastrointestinal hormones and esophageal mucosal damage in RE patients with stagnant heat in liver and stomach,it has better clinical effect and good safety.
