摘要
目的 探讨前庭蜗神经微血管压迫综合征患者的临床特征及MRI影像学表现。方法 选取新乡市某医院2020年5月—2022年1月收治的36例前庭蜗神经微血管压迫综合征患者作为研究对象,分析患者临床特征以及MRI影像学表现。结果 36例患者均存在眩晕反复持续发作现象。眩晕可在静息状态与任何头位改变状态时发作,任何头位改变均可导致患者出现眩晕等症状,无特殊激发头位。另有眩晕伴呕吐1例,眩晕伴恶心2例,运动耐受不良23例,耳鸣5例,听力下降6例,头位变动诱发眩晕31例,眼震5例。36例患者经磁共振检查后均可见前庭蜗神经与血管接触或受压。其中神经血管压迫部位单侧33例,双侧3例;压迫类型Ⅰ型18例,Ⅱ型3例,Ⅲ型13例,Ⅳ型2例;责任血管为小脑下前动脉32例,小脑下后动脉4例。结论 临床可通过前庭蜗神经微血管压迫综合征患者临床表现及MRI影像学表现排除因其他原因导致的眩晕,以保证确诊后及时进行治疗,从而改善患者预后。
Objective To analyze the clinical features and MRI findings of patients with microvascular compression of the vestibulocochlear nerve.Methods A total of 36 patients with microvascular compression of the vestibulocochlear nerve who were admitted to the hospital from May 2020 to January 2022 were selected as the research subjects.The patients’clinical characteristics and MRI findings were analyzed.Results All of the 36 patients had recurrent and persistent vertigo which occurred in the resting state and whenever head position changed.There was 1 case of vertigo with vomiting,2 cases of vertigo with nausea,23 cases of exercise intolerance,5 cases of tinnitus,6 cases of hearing loss,31 cases of vertigo induced by head position change,and 5 cases of nystagmus.MR imaging showed that the vestibulocochlear nerve was in contact with blood vessels or was compressed.There were 33 cases of unilateral neurovascular compression and 3 cases of bilateral neurovascular compression.Type I,Ⅱ,Ⅲand IV compression were observed in 18 cases,3 cases,13 cases and 2 cases,respectively.Anterior inferior cerebellar artery was the responsible blood vessel in 32 cases and posterior inferior cerebellar artery was the responsible blood vessel in 4 cases.Conclusion The clinical features and MRI findings of patients with microvascular compression of the vestibulocochlear nerve can help to rule out vertigo caused by other factors,and ensure timely treatment after diagnosis,thereby improving the prognosis.
