烧伤整形患者血清Toll样受体4和髓样细胞触发受体1表达水平及影响因素分析 被引量:2

摘要 目的:探讨烧伤后整形患者血清Toll样受体4(TLR4)和髓样细胞触发受体1(TREM1)表达水平及其影响因素。方法:以烧伤整形患者58例(烧伤整形组)和健康体检者50例(健康对照组)作为研究对象。采用酶联免疫吸附法(ELISA)检测所有研究对象血清TLR4和TREM1表达水平。采用Pearson法分析两者的相关性。采用单因素和多因素Logistic回归分析烧伤整形患者血清TLR4和TREM1水平的影响因素。结果:烧伤整形组血清TRL4和TREM1水平显著高于健康对照组(均P<0.05)。烧伤整形患者血清TRL4和TREM1水平呈正相关(r=0.613,P<0.05)。单因素和Logistic回归分析结果表明,增生性瘢痕、整形手术时间≥180 min、抗菌药物种类>2种、术区残余创面、术后感染是影响烧伤整形患者血清TLR4水平的独立危险因素,而增生性瘢痕、整形手术时间≥180 min、术区残余创面、术后感染是影响烧伤整形患者血清TREM1水平的独立危险因素(均P<0.05)。结论:烧伤整形患者血清TLR4和TREM1表达水平升高。它们能够反映全身炎症程度,并与多种因素密切相关,应控制整形手术时间,提高创面愈合,预防术后感染,合理使用抗生素。 Objective:To investigate the expression levels of Toll like receptor 4(TLR4)and triggering receptor expressed on myeloid cell 1(TREM1)in burn and plastic surgery patients and their influencing factors.Methods:Fifity-eight burn plastic patients(burn plastic group)and Fifity healthy subjects(healthy control group)were taken as the research objects.The expression levels of TLR4 and TREM1 in serum of all subjects were detected by ELISA,and the correlation between them was analyzed by Pearson method.Univariate and multivariate Logistic regression were used to analyze the influencing factors of serum TLR4 and TREM1 in burn and plastic surgery patients.Results:The levels of serum TRL4 and TREM1 in burn and plastic surgery group were significantly higher than those in healthy control group(all P<0.05).Perason analysis showed that there was a significant positive correlation between the levels of serum TRL4 and TREM1 in burn and plastic surgery patients(r=0.613,P<0.05).Univariate and multivariate Logistic regression analysis showed that hypertrophic scar,plastic surgery time≥180 minutes,types of antibiotics>2 kinds,residual wound in the operation area and postoperative infection were independent risk factors affecting the serum TLR4 level of burn and plastic surgery patients,while hypertrophic scar,plastic surgery time≥180 minutes,residual wound in the operation area and postoperative infection were independent risk factors affecting the serum TREM1 level of burn and plastic surgery patients(all P<0.05).Conclusion:The expression levels of TLR4 and TREM1 in burn and plastic patients are increased.They can reflect the degree of systemic inflammation and are closely related to many factors.We should control the time of plastic surgery,improve wound healing,prevent postoperative infection,reduce the variety of antibiotics.
