摘要
目的:分析血浆D-二聚体与胃癌患者预后的关系。方法:通过计算机检索PubMed文献数据库、根哥学术(Geen Medical)、考克兰图书馆(Cochrane Library)、万方数据知识服务平台(Wanfang Data)、中国知网(CNKI)等国内外数据库中所有探讨血浆D-二聚体与胃癌患者预后关系的文献,检索时间为建库至2021年12月31日。采用Rev Man 5.4统计学软件进行分析,使用具有95%置信区间(CI)的风险比(HR)来评估血浆D-二聚体与胃癌患者总生存时间(OS)、无病生存时间(DFS)或无复发生存时间(RFS)的关系。结果:纳入6项研究,共计3509例胃癌患者。Meta分析结果显示,D-二聚体水平较高的胃癌患者OS(HR=1.57,95%CI:1.21~2.03,P=0.0006)及RFS或DFS(HR=2.26,95%CI:1.70~3.00,P<0.00001)均缩短。根据国家、样本量、D-二聚体临界值及纽卡斯尔-渥太华量表(NOS)评分分组进行亚组分析,结果显示D-二聚体水平较高的胃癌患者OS明显缩短(均P<0.05)。结论:血浆D-二聚体升高与胃癌患者OS、RFS或DFS缩短相关,可以作为胃癌患者的预后指标。
Objective:To analyze the relationship between plasma D-dimer and prognosis of gastric cancer patients.Methods:By searching PubMed,Geen Medical,Cochrane Library,Wanfang Data,CNKI and other domestic and foreign databases for all relevant studies on the relationship between plasma D-dimer and the prognosis of patients with gastric cancer,the retrieval time was from the establishment of the database to December 31,2021.Rev Man software(version 5.4)was used for data analysis.The hazard ratio(HR)with 95%confidence interval(CI)was used to evaluate the correlation between plasma D-dimer and overall survival(OS)and recurrence free survival(RFS)or disease-free survival(DFS)in gastric cancer patients.Results:Six studies were finally included,including 3509 patients with gastric cancer.Meta analysis showed that OS(HR=1.57,95%CI:1.21 to 2.03,P=0.0006)and RFS or DFS(HR=2.26,95%CI:1.70~3.00,P<0.00001)were shortened in patients with gastric cancer with higher D-dimer level.Subgroup analysis according to the country,sample size,D-dimer critical value and NOS score showed that the higher level of D-dimer was related to the shortening of OS in patients with gastric cancer(all P<0.05).Conclusion:The increase of plasma D-dimer is associated with shortening of OS,RFS or DFS in patients with gastric cancer and can be used as a prognostic indicator in patients with gastric cancer.
