摘要
目的 研究IQon光谱CT对不同直径肺结节的诊断价值。方法 研究对象选取经临床检查发现实性肺部结节的患者70例,总计实性肺部结节72个,分别对其进行传统增强CT和IQon光谱CT检查,后由3名影像学医师判读,所有结节均有病理活检诊断,比较2种方法在不同直径肺部结节中与病理金标准诊断结果符合率,并对IQon光谱CT参数进行最佳阈值进行分析。结果 病理诊断恶性肺部结节48个,良性肺部结节24个。IQon光谱CT与病理诊断符合率为94.4%,相较于传统增强CT的77.8%符合率差异有统计学意义(P<0.001),进一步分析在直径<1 cm的恶性肺部结节的诊断中,IQon光谱CT有明显优势(P=0.036)。根据IQon光谱CT平均碘浓度、标准化碘浓度以及有效原子序数参数建立ROC曲线,动脉期平均碘浓度和动脉期标准化碘浓度诊断性能高。结论 IQon光谱CT对肺部结节具有较高的诊断价值,尤其对直径较小的恶性肺部结节诊断准确率更高,并且动脉期平均碘浓度和动脉期标准化碘浓度的诊断参考价值最高。
Objective To study the diagnostic value of IQon spectral CT for pulmonary nodules of different diameters.Methods The study subjects selected 70 patients with solid lung nodules found through clinical examination,a total of 72 solid lung nodules,and they were examined by traditional enhanced CT and IQon spectral CT respectively,and then interpreted by three imaging physicians,All nodules have pathological biopsy diagnosis,compare the two methods in different diameter pulmonary nodules with the pathological gold standard diagnosis results in accordance with the rate,and analyze the best threshold of IQon spectral CT parameters.Results Forty-eight malignant pulmonary nodules and 24 benign pulmonary nodules were pathologically confirmed.The coincidence rate of IQon spectral CT and pathological diagnosis is 94.4%,which is significantly different from the 77.8%coincidence rate of traditional enhanced CT(P<0.001).Further analysis is used in the diagnosis of malignant pulmonary nodules with a diameter of<1 cm.CT has obvious advantages(P=0.036).According to the IQon spectrum CT average iodine concentration,standardized iodine concentration and effective atomic number parameters to establish ROC curve,the average iodine concentration in the arterial phase and the standardized iodine concentration in the arterial phase have high diagnostic performance.Conclusion IQon spectral CT has a high diagnostic value for pulmonary nodules,especially for malignant pulmonary nodules with a smaller diameter.The diagnostic accuracy is higher,and the average iodine concentration in the arterial phase and the standardized iodine concentration in the arterial phase are diagnostic reference values Highest.
