摘要
目的 探讨喜炎平注射液治疗不同证型乙型流感病毒肺炎的有效性与安全性。方法 回顾性分析山东中医药大学附属医院儿科2019年1月至2020年12月收治的137例乙型流感病毒肺炎患儿的临床资料,依据病程记录结合中医辨证标准分为风热组(41例)、湿热组(48例)、痰热组(48例)。3组患儿均静脉滴注喜炎平注射液10 mg/(kg·d),剂量不超过250 mg。结果 137例患儿总有效率为83.21%(114/137);风热组总有效率为95.12%,显著高于湿热组的79.17%和痰热组的77.08%(P <0.05)。3组患儿治疗后的主症积分均较治疗前显著降低(P <0.05),但组间无显著差异(P> 0.05)。退热时间长短依次为痰热组>湿热组>风热组,且组间差异显著(P <0.05);住院天数、用药天数、罗音消失时间和咳嗽消失时间组间差异均显著(P <0.05),经两两比较,湿热组显著长于风热组和痰热组(P <0.05),风热组和痰热组比较无显著差异(P> 0.05)。风热组、湿热组、痰热组治疗期间的合并感染和并发症发生情况均相当(51.22%比50.00%比45.83%,17.07%比25.00%比12.50%,P> 0.05);所有患儿治疗期间均未发生药品不良反应。结论 喜炎平注射液治疗风热闭肺证、湿热闭肺证、痰热闭肺证乙型流感病毒肺炎的临床疗效均较好,可有效改善患儿的临床症状与体征,且安全性高。
Objective To investigate the efficacy and safety of Xiyanping Injection in the treatment of children with different syndromes of influenza B virus pneumonia.MethodsThe clinical data of 137 children with influenza B virus pneumonia admitted to the Pediatrics Department of the Affiliated Hospital of Shandong University of Traditional Chinese Medicine from January 2019to December 2020 were analyzed retrospectively.According to the course records and the Chinese medical syndrome differentiation standards,the above children were divided into the wind-heat group (41 cases),the damp-heat group (48 cases) and the phlegmheat group (48 cases).Children in the three groups were given 10 mg/(kg·d) of Xiyanping Injection intravenously at a dose of no more than 250 mg.ResultsThe total effective rate in the 137 children was 83.21%(114/137),and that in the wind-heat group was 95.12%,which was significantly higher than 79.17%in the damp-heat group and 77.08%in the phlegm-heat group(P<0.05).After treatment,the main symptom scores in the three groups were significantly lower than those before treatment(P<0.05),while the main symptom score in the wind-heat group was similar to that in the damp-heat group and phlegmheat group (P>0.05).The antipyretic time was longest in the phlegm-heat group,followed by the damp-heat group and the wind-heat group,and there was a significant difference among the three groups (P<0.05).There were significant differences in the hospitalization days,medication days,rale disappearance time and cough disappearance time among the three groups (P<0.05).Pairwise Comparison results showed that the above four indexes in the damp-heat group were significantly longer than those in the wind-heat group and the phlegm-heat group (P<0.05),while those in the wind-heat group were similar to those in the phlegm-heat group (P>0.05).During the treatment,the incidence of co-infection and complications in the wind-heat group was similar to that in the damp-heat group and phlegm-heat group (51.22%vs.50.00%vs.45.83%,17.07%vs.25.00%vs.12.50%,P>0.05).During the treatment,no adverse drug reactions occurred in all children.ConclusionXiyanping Injection is effective and safe in the treatment of influenza B virus pneumonia with wind-heat closing lung syndrome,damp-heat closing lung syndrome and phlegm-heat closing lung syndrome,which can effectively improve the clinical symptoms and signs of children.
