摘要
目的 探讨α-硫辛酸联合甲钴胺在糖尿病周围神经病变治疗中的疗效及对患者氧化应激、神经传导功能的影响。方法 选取2020年1月—2021年12月于徐州市贾汪区人民医院接受治疗的104例糖尿病周围神经病变患者,采取随机数表法分为两组,各52例。对照组采用甲钴胺治疗,观察组采用甲钴胺联合α-硫辛酸治疗,比较两组患者临床有效率,神经传导功能(正中、腓总神经的SNCV、MNCV)、氧化应激指标(SOD、GSHPx)水平以及不良反应发生情况。结果 治疗后观察组患者的正中神经MNCV、SNCV和腓总神经的MNCV、SNCV均高于对照组,差异有统计学意义(t=8.956、7.795、7.442、8.407,P<0.05);治疗后,观察组患者的SOD水平、GSH-Px水平均高于对照组,差异有统计学意义(t=11.909、9.951,P<0.05);治疗后观察组患者总有效率为96.15%高于对照组的78.85%,差异有统计学意义(χ^(2)=7.121,P<0.05);治疗期间,两组患者出现食欲不振、腹痛腹泻、恶心、呕吐及皮疹的总发生率分别为7.69%和11.54%,差异无统计学意义(χ^(2)=0.443,P>0.05)。结论α-硫辛酸联合甲钴胺在糖尿病周围神经病变治疗中疗效较好,可有效提高神经传导速度,改善氧化应激水平,且治疗期间不良反应较少,值得推广应用。
Objective To investigate the efficacy of α-lipoic acid combined with methylcobalamin in the treatment of diabetic peripheral neuropathy and its effect on oxidative stress and nerve conduction function in patients.Methods A total of 104 patients with diabetic peripheral neuropathy who received treatment from January 2020 to December 2021 admitted to the People’s Hospital of Jiawang District of Xuzhou City were selected.Divided into two groups by random number method,each with 52 cases.The control group was treated with mecobalamin,and the observation group was treated with mecobalamin combined with α-lipoic acid.The clinical efficacy,nerve conduction function(SNCV and MNCV of the median and common peroneal nerve),oxidative stress indicators(SOD,GSH-Px)and the occurrence of adverse reactions were compared between the two groups.Results After treatment,the median nerve MNCV,SNCV and MNCV and SNCV of common peroneal nerve in the observation group were higher than those in the control group,and the differences were statistically significant(t=8.956,7.795,7.442,8.407,P0.05).Conclusion α-lipoic acid combined with methylcobalamin has a good curative effect in the treatment of diabetic peripheral neuropathy,which can effectively increase the nerve conduction velocity and improve the level of oxidative stress,with less adverse reactions during treatment,worthy of promotion and application.
