摘要
目的:探究低分子肝素钠联合硫酸镁、盐酸拉贝洛尔治疗妊娠期高血压综合征(PIH)的疗效以及安全性。方法:选取2019年5月至2020年5月期间南阳市中心医院收治的PIH患者98例,将受试者分为观察组、对照组,每组49例。对照组给予常规硫酸镁和盐酸拉贝洛尔治疗;观察组在对照组基础上联合低分子肝素钠治疗。治疗后比较两组患者收缩压(SBP)、舒张压(DBP)、凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、纤维蛋白原(FIB)、D-二聚体(D-D2)以及24 h尿蛋白(24 h UP)、血尿素氮(BUN)、肌酐(Scr),并记录不良事件发生率。结果:两组患者血压均明显降低,且观察组降低更显著(P<0.05);24 h UP、BUN、Scr、FIB、D-D2相比治疗前亦降低;凝血功能指标PT、APTT均出现上升(P<0.05),观察组上升更显著(P<0.05);观察组不良事件发生率低于对照组(6.12% vs 24.49%,P<0.05);两组患者用药不良反应率差异无统计学意义(10.20% vs 14.29%,P>0.05)。结论:低分子肝素钠联合硫酸镁、盐酸拉贝洛尔治疗对缓解PIH临床症状、减轻肾功能损伤效果显著,且妊娠不良发生风险低,安全性高。
Objective:To investigate the efficacy and safety of low molecular weight heparin sodium combined with magnesium sulfate and labetalol hydrochloride in the treatment of pregnancy induced hypertension (PIH).Methods:98 patients with PIH treated in Nanyang Central Hospital from May 2019 to may 2020 were selected.The subjects were divided into observation group and control group,with 49 cases in each group.The control group was treated with conventional magnesium sulfate and labetalol hydrochloride.The observation group was treated with low molecular weight heparin sodium on the basis of the control group.After treatment,systolic blood pressure (SBP),diastolic blood pressure (DBP),prothrombin time (PT),activated partial thromboplastin time (APTT),fibrinogen(FIB),d-dimer(D-D2),24 h urinary protein (24 h UP),blood urea nitrogen (BUN) and creatinine (SCR) were compared between the two groups,and the incidence of adverse events was recorded.Results:The blood pressure of the two groups decreased significantly,the decrease in the observation group was more significant (P0.05).Conclusion:The combination of low molecular weight heparin sodium combined with magnesium sulfate and labetalol hydrochloride has a significant effect on alleviating the clinical symptoms of PIH and reducing the injury of renal function,with low risk of adverse pregnancy and high safety.
