摘要
目的探讨血流限制低强度有氧训练对2型糖尿病(T2DM)患者糖脂代谢和炎症指标的影响。方法54名T2DM患者分为低强度有氧训练组(LI,n=18)、高强度有氧训练组(HI,n=18)和血流限制低强度有氧训练组(LI-BFR,n=18),HI组进行70%储备心率(HRR)间歇踏车训练;LI组和LI-BFR组强度为40%HRR,且LI-BFR组施加50%动脉闭塞压(AOP)的血流限制。频率为训练5min休息1min为1组,每次6组。每周进行3次,持续12周。对比各组训练前后血糖、血脂和炎症指标的变化。结果12周干预后,各组的FPG、TC、TG、FFA,HI组和LI-BFR组FINS,LI-BFR组HbA1c和HI组TNF-α均显著降低(所有p<0.05),且LI-BFR组QUICKI,LI-BFR组和HI组HDL-C,IL-6均显著上升(p<0.05)。但FPG、HbA1c、HOMA-IR和FFA变化表现出LI-BFR组取得更佳效果(p<0.05)。结论3种训练方式均能够对T2DM患者糖脂代谢产生一定影响,但血流限制低强度有氧训练降低FPG、HbA1c、HOMA-IR和FFA效果更显著;3种训练方式能够降低TNF-α水平,且血流限制低强度有氧训练和高强度有氧训练效果类似。
Objective To investigate the effects of blood flow-restriction low-intensity aerobic training on glucose and lipid metabolism and inflammatory markers in patients with type 2 diabetes mellitus(T2DM).Methods 54 T2DM patients were divided into low-intensity aerobic training group(LI,n=18),high-intensity aerobic training group(HI group,n=18)and blood flow-restriction low-intensity aerobic training group(LI-BFR,n=18).They were subjected to 40%reserve heart rate(HRR)or 70%HRR intermittent treadmill training,5min/time,rest for 1min,repeat 6 groups,3 times/w,a total of 12w.In LI-BFR group,blood flow restriction of 50%arterial occlusive pressure(AOP)was applied on the basis of LI group.The frequency of training was 5min and rest was 1 group,and each time 6 groups were performed 3 times a week for 12 weeks to compare the changes of blood glucose,lipid and inflammation indexes before and after training.Results After 12 weeks intervention,FPG,TC,TG,FFA in all groups,FINS,HOMA-IR in HI group and LI-BFR group,HbA1c in LI-BFR group,TNF-αin HI group decreased significantly(all p<0.05),QUICKI in LI-BFR group,HDL-C,IL-6 in LI-BFR group and HI group increased significantly(all p<0.05).But FPG,HbA1c,HOMA-IR and FFA only showed significant difference(all p<0.05).It showed that the LI-BFR group achieved better results.Conclusion The three training methods can have a certain impact on glucose metabolism and lipid metabolism in T2DM patients,but blood flow-restriction low-intensity aerobic training can reduce FPG,HbA1c,HOMA-IR and FFA more significantly Three training methods can reduce TNF-α,and the effect of blood flow-restriction low-intensity aerobic training is similar to that of high-intensity aerobic training.
