摘要
目的研究CD4^(+)CD25^(+)Foxp3^(+)调节性T细胞(Treg)及其相关因子在梅尼埃病中的临床意义。方法回顾性纳入2019年8月至2021年5月收治的45例梅尼埃病患者(其中缓解期13例,发作期32例;单侧36例,双侧9例;听力分期1期3例、2期20例、3期16例、4期6例),以同期健康人群30例作为对照组。采用流式细胞术检测两组患者外周血CD4^(+)CD25^(+)Foxp3^(+)Treg细胞百分比及转化生长因子-β(TGF-β)、叉头状转录因子p3(Foxp3)mRNA相对表达;酶联免疫吸附法(ELISA)检测血清白细胞介素-10(IL-10)水平;分析CD4^(+)CD25^(+)Foxp3^(+)Treg细胞及其相关细胞因子TGF-β、Foxp3、IL-10与病情严重程度、不同听力分期和单双侧患耳的关系;Pearson相关性分析CD4^(+)CD25^(+)Foxp3^(+)Treg细胞表达与IL-10、TGF-β、Foxp3表达的相关性。结果梅尼埃病组患者外周血CD4^(+)CD25^(+)Foxp3^(+)Treg细胞百分比、外周血Foxp3 mRNA相对表达及血清IL-10水平明显低于健康对照组,外周血TGF-βmRNA相对表达则明显高于健康对照组,差异有统计学意义(均为P<0.05)。不同听力分期及单双侧梅尼埃病患者外周血CD4^(+)CD25^(+)Foxp3^(+)Treg细胞百分比及相关细胞因子表达组间差异均无统计学差异(P>0.05)。梅尼埃病患者CD4^(+)CD25^(+)Foxp3^(+)Treg细胞百分比与外周血TGF-β相对表达呈负相关(r=-0.704,P<0.05),与血清IL-10水平(r=0.679)和外周血Foxp3相对表达(r=0.725)呈正相关(P<0.05)。结论梅尼埃病患者外周血CD4^(+)CD25^(+)Foxp3^(+)Treg细胞及相关细胞因子TGF-β、Foxp3、IL-10异常表达,可能与患者体内免疫抑制减弱有关,Treg细胞调控的细胞免疫可能是梅尼埃病的发病机制之一。
Objective To study the clinical value of CD4^(+)CD25^(+)Foxp3^(+)regulatory T cells(Treg)and the related factors in Meniere s disease.Methods Forty-five patients with Meniere s disease admitted to our hospital from August 2019 to May 2021 were retrospectively enrolled,and 30 healthy people without history of ear and nose diseases during the same period were included as controls.The percentage of CD4^(+)CD25^(+)Foxp3^(+)Treg cells and mRNA relative expressions of transforming growth factor-β(TGF-β)and fork-head transcription factor P3(Foxp3)in peripheral blood of 2 groups were detected by flow cytometry.Serum interleukin-10(IL-10)level was determined by enzyme-linked immunosorbent assay(ELISA).The relationship between CD4^(+)CD25^(+)Foxp3^(+)Treg cells and their related cytokines TGF-β,Foxp3,IL-10 and the severity of the disease,different hearing grade and unilateral and bilateral ear disease was analyzed.Pearson correlation analysis of the expression of CD4^(+)CD25^(+)Foxp3^(+)Treg cells and the expression of IL-10,TGF-β,Foxp3.Results The percentage of CD4^(+)CD25^(+)Foxp3^(+)Treg cells in peripheral blood,the relative expression of Foxp3 mRNA in peripheral blood and the level of serum IL-10 in Meniere s disease group were significantly lower than those in healthy control group.Mean while,the relative expression of TGF-βmRNA in peripheral blood was significantly higher than that in healthy control group,the differences were statistically significant(P<0.05).There were no significant differences in the percentage of CD4^(+)CD25^(+)Foxp3^(+)Treg cells and the expression of related cytokines in peripheral blood of patients with Meniere s disease(P>0.05).The percentage of CD4^(+)CD25^(+)Foxp3^(+)Treg cells in patients with Meniere s disease was negatively correlated with the relative expression of TGF-βin peripheral blood(r=-0.704,P<0.05),and positively correlated with the level of IL-10 in serum and the relative expression of Foxp3 in peripheral blood(r=0.679 and 0.725,P<0.05).Conclusion The abnormal expression of CD4^(+)CD25^(+)Foxp3^(+)Treg cells and TGF-β,Foxp3 and IL-10 in peripheral blood of patients with Meniere s disease was closely related to the progression of the disease.
