摘要
目的:分析子宫脱垂疾病特点,评价改良腹腔镜子宫悬吊术治疗的术后效果。方法:选择2019年5月~2020年2月本院妇科收治的94例子宫脱垂疾病患者,经医院伦理委员会审核批准,进行对照组、观察组随机法分组治疗。对照组(n=47)患者采取传统子宫切除术,观察组(n=47)患者采取改良腹腔镜子宫悬吊术。比较经不同术式治疗后子宫脱垂患者手术指标、术后并发症、生活质量,以盆底功能障碍问卷(pelvic floor distress inventory-short form 20,PFDI-20)评价患者的生活质量。结果:治疗后,与对照组比较,观察组子宫脱垂患者手术、术后恢复用时更短(P<0.05),术中出血量更少(P<0.05),压力性尿失禁、感染并发症发生率(4.25%)更低(P<0.05),术后6个月PFDI-20评分(12.50±4.50)更低(P<0.05)。结论:比较传统子宫切除术,改良腹腔镜子宫悬吊术治疗子宫脱垂整体优势突出,具有微创、损伤小、恢复快、安全性高等优势,有效改善了患者的生活质量,是子宫脱垂疾病的理想术式。
Objective:To analyze the characteristics of uterine prolapse disease and evaluate the postoperative effect of improved laparoscopic uterine sling.Methods:A total of 94 patients with uterine prolapse disease admitted to our hospital during May 2019 to February 2020 were selected.After the approval of the ethics committee of the hospital,the patients were randomly divided into control group and observation group for treatment.Patients in the control group(n=47)received conventional hysterectomy,and patients in the observation group(n=47)received modified laparoscopic uterine sling.The surgical indicators,postoperative complications and quality of life of patients with uterine prolapse treated with different operation methods were compared.The quality of life of the patients was evaluated with the pelvic floor distress inventory-short form 20(PFDI-20)questionnaire.Results:After treatment,compared with the control group,the duration of operation and postoperative recovery of patients with uterine prolapse in the observation group was shorter(P<0.05),the amount of intraoperative blood loss was less(P<0.05),and the incidence of stress urinary incontinence and infection complications was lower(4.25%)(P<0.05).6 months after the operation,the score of PfDI-20(12.50±4.50)was even lower(P<0.05).Conclusion:Compared with traditional hysterectomy,modified laparoscopic uterine sling has outstanding overall advantages in the treatment of uterine prolapse,which has the advantages of minimally invasive,small injury,quick recovery and high safety,and effectively improves the quality of life of patients.It is an ideal surgical method for uterine prolapse disease.
