摘要
目的 探讨水蛭粉外用对大鼠慢性难愈性创面修复愈合的促进作用及其机制。方法 32只雄性SD大鼠背部制造全层皮肤缺损性创面,除正常对照组外,其余大鼠建立难愈性创面模型,随机分为模型组、水蛭粉外用组和西医对照组,观察各组大鼠创面造模后第3、7、11、15天的创面愈合率、愈合时间;在创面造模后第7天取各组大鼠背部创面肉芽组织标本,通过免疫组化及图像分析法检测肉芽组织中CD34、VEGF含量,用Western blot法检测肉芽组织中eNOS蛋白水平。结果 模型组大鼠各时间点创面愈合率均较正常对照组明显降低(P<0.01),创面愈合时间明显延长(P<0.01);水蛭粉外用组、西医对照组大鼠各时间点创面愈合率均较模型组显著提高(P<0.01,P<0.05),创面愈合时间均较模型组明显缩短(P<0.01)。创面造模后第7天,模型组大鼠创面肉芽组织中CD34、VEGF含量及eNOS蛋白水平均较正常对照组明显下降(P<0.01);水蛭粉外用组、西医对照组大鼠创面肉芽组织中CD34、VEGF含量及eNOS蛋白水平均较模型组明显上升(P<0.01);水蛭粉外用组大鼠创面肉芽组织中CD34、VEGF含量及eNOS蛋白水平上升较西医对照组更为明显(P<0.01)。结论 水蛭粉外用对大鼠慢性难愈性创面修复愈合有明显的促进作用,其机制与上调大鼠创面VEGF表达,使NO合成和分泌增加,从而诱导血管新生有关。
Objective To investigate the promoting effect and mechanism of leech powder on chronic refractory wound healing in rats.Methods The full-thickness skin defect wounds were made on the back of 32 male SD rats.Except addition to the normal control group,the other rats randomly divided into the model group,the leech powder external application group and the western medicine control group to establish the refractory wound model.The wound healing rate and healing time on the 3 rd,7 th,11 th and 15 th days after the wound modeling were observed.On the 7 th day after wound modeling,the granulation tissue samples of rats in each group were collected.The contents of CD34 and VEGF in granulation tissue were detected by immunohistochemistry and image analysis.The eNOS protein level in granulation tissue was detected by western blot.Results The wound healing rate of the model group at each time point was significantly lower than that of the normal control group(P<0.01),and the wound healing time was significantly prolonged( P<0.01).The wound healing rate of leech powder group and western medicine control group at each time point was significantly higher than that of model group(P<0.01,P<0.05),and the wound healing time was significantly shorter than that of model group(P<0.01).On the 7 th day after wound modeling,the contents of CD34,VEGF and eNOS protein in the granulation tissue of rats in the model group were significantly lower than those in the normal control group(P<0.01).The contents of CD34,VEGF and eNOS protein in granulation tissue of rats in leech powder group and western medicine control group were significantly higher than those in model group(P<0.01).The contents of CD34,VEGF and eNOS protein in granulation tissue of rats in leech powder group were higher than those in western medicine group(P<0.01).Conclusion The external application of leech powder has a significant promoting effect on the healing of chronic refractory wounds in rats.The mechanism is related to the up-regulation of VEGF expression in the wounds of rats,the increase of NO synthesis and secretion,and the induction of angiogenesis.
