摘要
心力衰竭是常见的心血管疾病之一,心脏收缩不同步在心力衰竭中有很高的发生率,是影响药物疗效并导致心功能进一步恶化的重要因素。从早期的右心室起搏到随后出现更有利于双心室同步收缩的双心室起搏,使数以万计的患者受益,但仍有30%的患者对这种非生理性起搏无应答。近年来通过心脏自身传导系统起搏的希浦系统起搏及与左心室起搏融合形成优化的心脏再同步化治疗开始运用临床,已有证据表明,其能进一步提高心脏电-机械同步性,最大化改善心脏运动不同步。相信随着技术的进步和循证医学的研究,有更多心力衰竭患者将从心脏再同步化治疗中获益。
Heart failure is one of the common cardiovascular diseases.Cardiac systolic asynchrony has a high incidence in heart failure,which is an important factor that affects the efficacy of drugs and leads to further deterioration of cardiac function.Tens of thousands of patients have benefited from the early right ventricular pacing to the subsequent emergence of biventricular pacing,which is more conducive to synchronous biventricular contraction,but 30% of patients are still unresponsive to this non-physiological pacing.In recent years,Hispurkinje conduction system pacing and optimized cardiac resynchronization therapy combined with left ventricular pacing have been used clinically.There has been evidence that it can further improve cardiac electromechanical synchronization and maximize the improvement of cardiac dyssynchrony.It is believed that with the progress of technology and the research of evidence-based medicine,more patients with heart failure will benefit from cardiac resynchronization therapy.
