摘要 目的:分析CT增强延时扫描用于肝脏肿瘤鉴别诊断的价值,为肝脏肿瘤诊断、治疗工作提供参考。方法:选择2019年2月~2020年3月本院收治的80例肝脏肿瘤疾病患者为观察对象,经CT增强延时扫描,获得伦理委员会批准、患者与家属知情同意,与手术病理结果进行对比分析。比较肝脏肿瘤患者的检查结果、手术病理结果,分析诊断符合率、误诊/漏诊情况以及影像特征。结果:80例肝脏肿瘤患者手术病理结果方面,包括原发性肝癌、肝内转移瘤、肝血管瘤、肝囊肿、肝脏局灶性结节性增生、肝脓肿、肝母细胞瘤,占比率分别为40%、15%、10%、15%、10%、5%、5%,诊断符合率100%。经CT增强延迟扫描后,诊断符合率为97.5%、误诊/漏诊率2.5%。经统计学计算,P>0.05。另外,分析图像特征,原发性肝癌动脉期结节强化、延迟期低密度状态,肝囊肿动脉期点状强化,各类肝脏肿瘤疾病影像特征有特异表现。结论:CT增强延时扫描技术鉴别诊断肝脏肿瘤准确率高,且检查创伤小、安全性高优势突出,为疾病早期治疗提供参考。 Objective:To analyze the value of CT enhanced time-lapse scanning in the differential diagnosis of liver tumors,and to provide references for the diagnosis and treatment of liver tumors.Methods:We selected 80 patients with liver tumor disease in our hospital from February 2019 to March 2020.After CT enhanced delayed scanning,approved by the ethics committee and informed consent of the patients and family members,they were compared with the surgical pathological results.Compare the examination results,surgical pathology results of patients with liver tumors,analyze the diagnosis coincidence rate,misdiagnosis/missed diagnosis,and imaging characteristics.Results:In terms of surgical and pathological results of 80 patients with liver tumors,including primary liver cancer,intrahepatic metastases,hepatic hemangioma,hepatic cyst,focal nodular hyperplasia of the liver,liver abscess,and hepatoblastoma,the proportions were respectively were 40%,15%,10%,15%,10%,5%,5%,and the diagnostic coincidence rate was 100%.After CT enhanced delayed scan,the diagnostic coincidence rate was 97.5%,and the misdiagnosis/missing diagnosis rate was 2.5%.After statistical calculation,P>0.05.In addition,analysis of image characteristics,primary liver cancer arterial phase nodule enhancement,delayed phase low-density state,hepatic cyst arterial phase point enhancement,imaging features of various liver tumor diseases have specific manifestations.Conclusion:CT enhanced time-lapse scanning technology has a high accuracy in the differential diagnosis of liver tumors,and the advantages of small inspection trauma and high safety are outstanding,which can provide references for early treatment of diseases.
