杏贝止咳颗粒联合沙美特罗替卡松吸入治疗对慢性阻塞性肺疾病稳定期患者血清乙酰胆碱酯酶及炎症指标水平变化的影响 被引量:6

摘要 目的 杏贝止咳颗粒联合沙美特罗替卡松吸入治疗对慢性阻塞性肺疾病(COPD)稳定期患者血清乙酰胆碱酯酶(AchE)及炎症指标水平变化的影响。方法 采用回顾性分析,选择2020年1月至2021年12月于中国人民解放军陆军第七十一集团军医院确诊的80例COPD稳定期患者,按照治疗方式不同分为COPD组(常规沙美特罗替卡松粉吸入剂治疗,n=40)和杏贝止咳颗粒组(在COPD组基础上实施杏贝止咳颗粒治疗,n=40)。对比分析两组患者肺功能指标[1秒用力呼气量(FEV1)、FEV1占预计值百分比(FEV1%)、FEV1/用力肺活量(FVC)]、血清AchE、炎症因子[超敏C反应蛋白水平(hs-CRP)、白细胞(WBC)数量、肿瘤坏死因子α(TNF-α)和白细胞介素-6(IL-6)]水平以及不良反应情况。结果 治疗前,两组肺功能指标比较,差异均无统计学意义(P> 0.05);与治疗前相比,治疗后,两组肺功能指标FEV1、FEV1%和FEV1/FVC均显著增加,差异均有统计学意义(P <0.05);治疗后,杏贝止咳颗粒组FEV1、FEV1/预计值和FEV1/FVC为(1.88±0.30) L、(69.33±3.97)%、(67.90±5.33)%,均显著高于COPD组[(1.53±0.21)L、(60.96±4.50)%、(62.47±4.65)%],差异均有统计学意义(P <0.05)。治疗后,杏贝止咳颗粒组血清hs-CRP、WBC、TNF-α和IL-6水平为(3.78±0.12)μg/mL、(10.62±1.24)×10~9/L、(178.69±44.23) ng/L、(433.09±59.19)ng/L,均显著低于COPD组[(10.33±1.53)μg/mL、(15.31±2.09)×10~9/L、(253.09±34.29) ng/L、(545.43±48.90)ng/L],差异均有统计学意义(P <0.05)。治疗后,杏贝止咳颗粒组AchE水平为(5.70±0.32) U/L,显著高于COPD组[(2.12±0.43) U/L],差异有统计学意义(P <0.05)。两组不良反应发生情况比较,差异无统计学意义(P> 0.05)。结论 在常规治疗基础上结合杏贝止咳颗粒治疗,可对慢性阻塞性肺疾病稳定期患者发挥显著治疗效果,具体体现在可显著改善患者肺功能,并进一步调控血清AchE以及炎症反应水平,且临床安全性较好。 Objective To explore the effect of Xingbei Zhike Granules combined with salmeterol and ficasone powder inhalation on the changes of serum acetylcholinesterase(AchE)and inflammatory indexes in patients with stable chronic obstructive pulmonary disease(COPD).Methods Eighty COPD patients diagnosed in the 71st Group Military Hospital of the Chinese People's Liberation Army Army from January 2020 to December 2021 were retrospectively selected and divided into Xingbei Zhike Granules group(n=40)and COPD group(n=40).Pulmonary function indicators[forced expiratory volume in 1 second(FEV1),percentage of FEV1 to predicted value(FEV1%),FEV1/forced vital capacity(FVC)],serum AchE,inflammatory factors hypersensitivity C-reactive protein(hs-CRP),white blood cell(WBC)counts,tumor necrosis factor alpha(TNF-α)and interleukin-6(IL-6)levels,and adverse reactions in the two groups were compared.Results Before treatment,there was no significant difference in pulmonary function indexes between the two groups(P>0.05);After treatment,FEV1,FEV1/predicted value and FEV1/FVC in Xingbei Zhike Granules group were(1.88±0.30)L,(69.33±3.97)%,(67.90±5.33)%,which were significantly higher than those in COPD group[(1.53±0.21)L,(60.96±4.50)%,(62.47±4.65)%],the differences were statistically significant(P<0.05).After treatment,the levels of serum hs-CRP,WBC,TNF-αand IL-6 in Xingbei Zhike Granules group were(3.78±0.12)μg/mL,(10.62±1.24)×10^(9)/L,(178.69±44.23)ng/L,(433.09±59.19)ng/L,were significantly lower than those in COPD group[(10.33±1.53)μg/mL,(15.31±2.09)×10^(9)/L,(253.09±34.29)ng/L,(545.43±48.90)ng/L],the differences were statistically significant(P<0.05).After treatment,the AchE level in Xingbei Zhike Granules group was(5.70±0.32)U/L,which was significantly higher than that in COPD group[(2.12±0.43)U/L],and the difference was statistically significant(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Combining Xingbei Zhike Granules on the basis of conventional treatment can exert a significant therapeutic effect on patients with stable chronic obstructive pulmonary disease.Specifically,it can significantly improve the lung function of patients,and further regulate the levels of serum AchE and inflammatory response,and the clinical safety is good.
