摘要
目的探讨采用切开复位锁定钢板内固定治疗“歪戴帽”型桡骨颈骨折的临床疗效。方法回顾性分析2016-11/2018-11月作者医院收治和采用切开复位锁定钢板内固定治疗“歪戴帽”型桡骨颈骨折的22例患者资料,其中男13例,女9例;年龄22~64岁,平均37.20岁;左侧损伤8例,右侧14例。其中15例合并不同程度肘部其他损伤,有8例为合并肘关节脱位。桡骨颈骨折按Broberg-Morrey改良的Mason分型:Ⅱ型3例,Ⅲ型11例,Ⅳ型8例。均采用肘关节外侧入路钢板内固定治疗,术后随访分析临床疗效。根据患者肘关节的疼痛程度、运动弧、稳定度及日常活动能力等情况,采用Mayo肘关节功能评分标准(Mayo elbow function score,MEPS)评估手术疗效。结果本组获随访12~34个月,平均17.90个月,所有22例患者桡骨颈骨折均获得临床愈合。未见骨折不愈合、桡骨头坏死、肘关节骨化性肌炎、关节僵硬等并发症。MEPS评估:优7例,良13例,中2例,优良率90.91%。结论采用切开复位锁定钢板内固定治疗“歪戴帽”型桡骨颈骨折可以获得满意的临床疗效。
Objective To explore the clinical effect of open reduction and internal fixation with steel plate in the treatment of"Crooked hat"type isolated crooked radial neck fractures.Methods A total of 22 cases of patients using the open reduction and internal fixation with"Crooked hat"type isolated radial neck fractures treated in auther′s hospital from November 2016 to November 2018 were analyzed by retrospective analysis.There were 13 males and 9females among the patients which aged from22 to 64 years,with an average age of 37.20;8 cases were injured on the left and 14 cases were on the right.Fifteen patients were combined with other injuries to the elbow,and 8 of them had dislocation of the elbow joint.Radial neck fractures were classified by Broberg-Morrey′s modified Mason classification:3 cases of typeⅡ,11 cases of typeⅢand 8 cases of typeⅣ.All patients were treated by internal fixation with lateral elbow approach,and postoperative follow-up analysis of clinical efficacy was carried out.The surgical efficacy was assessed by Mayo elbow function score(MEPS)based on degree of elbow pain,motor arc,stability and ability to perform daily activities.Results The group was followed up for 12-34 months with an average of 17.90 months.All 22 patients with radial neck fractures obtained clinical healing.There were no complications such as fracture nonunion,radial head necrosis,elbow ossifying myositis and joint stiffness.MEPS was used to evaluate the effects of the operation:7 cases were excellent,13 cases were good,and 2 cases were moderate,excellent and good rate was 90.91%.Conclusion The use of open reduction plate internal fixation to treat"Crooked hat"type isolated radial neck fractures can obtain satisfactory clinical efficacy.
