摘要
目的:探讨使用腔镜手术的方式和传统的开刀手术对于肝细胞癌的疗效区别。方法:主要以昆明医科大学第二附属医院接收的肝癌患者为研究对象,以手术方式的不同,将其划分为观察组(腹腔镜手术40例)及对照组(传统手术40例)。比较两组手术进展及术后康复情况、实验室检查指标及各种并发症出现情况。结果:与对照组相比,观察组手术耗时更长、术中出血量更少、下床时间更早、胃肠道通气时间更短、住院天数更短,差异有统计学意义(P<0.05);在术后肝功能的指标中,总胆红素(TBIL)差异无统计学意义(P>0.05),但是天冬氨酸氨基转移酶(AST)及丙氨酸氨基转移酶(ALT)差异有统计学意义(P<0.05);出现并发症的情况为观察组17.5%,对照组为25%,差异无统计学意义(P>0.05)。结论:使用腹腔镜切除肝脏在改善肝细胞癌术中情况及术后恢复方面有重要地位。
Objective To explore the efficacy of endoscopic surgery and the traditional surgery for hepatocellular carcinoma.Method The patients with liver cancer accepted by the Second Affiliated Hospital of Kunming Medical University were divided into observation group(40 cases of laparoscopic surgery) and control group(40 cases of traditional surgery) according to different surgical methods.The surgical progress and postoperative recovery conditions, blood and biochemical conditions and various complications were compared between the twogroups.Results Compared with the control group, the observation group took longer operation time, less intraoperative bleeding, earlier time to get out of bed, shorter gastrointestinal ventilation time, and shorter hospitalization days. The difference was statistically significant(P<0.05);Among the indicators of postoperative liver function, there was no significant difference in total bilirubin(P>0.05),However, the difference between aspartate aminotransferase and alanine aminotransferase was statistical significant(P<0.05);Complications were 17.5% in the observation group and 25% in the control group, and the difference was not statistically significant(P>0.05).Conclusion The use of laparoscopic resection of the liver has an important role in improving hepatocellular carcinoma surgery and postoperative recovery of hepatocellular carcinoma.
