摘要
目的分析ZEB1、miR-200、血管内皮生长因子(VEGF)在复发性流产(RSA)患者绒毛组织中的表达及临床意义。方法选取妇产科门诊接诊的100例RSA患者,将其设置为观察组,同时,选出另外100例同期正常妊娠、要求人工终止妊娠的妇女,将其设置为对照组,检测、对比两组ZEB1、miR-200、VEGF胎盘绒毛组织中的表达,并分析上述3种指标在早期RSA患者、晚期RSA患者绒毛组织中、血清中的各自表达量。结果观察组的miR-200表达明显高于对照组,ZEB1、VEGF表达明显低于对照组,差异有统计学意义(P<0.05)。在绒毛组织中观察组ZEB1、miR-200、VEGF表达量分别为:0.82±0.07、1.56±0.23、0.79±0.08,对照组ZEB1、miR-200、VEGF表达为1.20±0.10、0.92±0.16、1.06±0.17;两组比较差异有统计学意义(P<0.05)。血清中观察组ZEB1、miR-200、VEGF表达量分别为:0.84±0.19、1.82±0.34、1.02±0.20,对照组ZEB1、miR-200、VEGF表达为:1.27±0.25、0.93±0.12、4.93±0.13;两组比较差异有统计学意义(P<0.05)。与对照组早期RSA、晚期RSA相比,观察组早期RSA、晚期RSA绒毛组织中、血清中miR-200表达量升高,ZEB1、VEGF表达量明显降低(P<0.05)。与观察组早期RSA相比,晚期RSA绒毛组织中、血清中miR-200表达量降低,ZEB1、VEGF表达量升高(P<0.05)。相关性分析显示,ZEB1、miR-200呈负相关(r=-0.349,P=0.004);miR-200、VEGF呈负相关(r=-0.486,P=0.001);ZEB1、VEGF呈正相关(r=0.654,P=0.001)。受试者工作特征(ROC)曲线分析显示,与ZEB1、miR-200、VEGF单项诊断相比,三项联合对RSA诊断价值较高(P<0.05)。结论RSA患者绒毛细胞组织中、血清中ZEB1、VEGF表达会异常降低,miR-200表达会异常升高,且ZEB1、miR-200、VEGF表达水平与RSA发生有直接关系,ZEB1、miR-200、VEGF三者表达之间存在相关性,联合诊断可以预估RSA的发生。
Objective To analyze the expression and clinical significance of ZEB1,miR-200,and vascular endothelial growth factor(VEGF)in the villi tissues of patients with recurrent miscarriage(RSA).Methods A hundred RSA patients admitted to our hospital from January 2019 to January 2021 in the obstetrics outpatient department were selected as the study group.At the same time,another 100 women who have normal pregnancy during the same period and require artificial termination of pregnancy were selected as control group.The expression of ZEB1,miR-200,and VEGF in placental villi of the two groups were assessed and compared,and analyze in the villi tissues and serum of early RSA patients and advanced RSA patients.Results The expression of miR-200 in the study group was significantly higher than that in the control group,and the expression of ZEB1 and VEGF was significantly lower than that in the control group.The villus expression levels of ZEB1,miR-200,and VEGF in the study group were 0.82±0.07,1.56±0.23,and 0.79±0.08,respectively;the villus expression of ZEB1,miR-200,and VEGF in the control group were 1.20±0.10,0.92±0.16,and 1.06±0.17,respectively.The serum expression of ZEB1,miR-200,and VEGF in the study group were 0.84±0.19,1.82±0.34,and 1.02±0.20,respectively;the serum expression of ZEB1,miR-200 and VEGF in the control group were 1.27±0.25,0.93±0.12,and 4.93±0.13,respectively.The differences were statistically significant(P<0.05).Compared with early RSA and late RSA in the control group,the expression levels of miR-200 in villi and serum of early RSA and late RSA in the study group were significantly increased,while the expression levels of ZEB1 and VEGF were significantly reduced(P<0.05).Compared with early RSA in the study group,the expression levels of miR-200 in villi and serum of late RSA were significantly reduced,and the expression levels of ZEB1 and VEGF were significantly increased(P<0.05).Correlation analysis showed that ZEB1 and miR-200 were significantly negatively correlated(r=-0.349,P=0.004);miR-200 and VEGF were significantly negatively correlated(r=-0.486,P=0.001);and ZEB1 and VEGF were significantly positively correlated(r=0.654,P=0.001).ROC curve analysis showed that compared with the single diagnosis of ZEB1,miR-200,or VEGF,the combination of the three had a higher diagnostic value for RSA(P<0.05).Conclusion In patients with RSA,the expression levels of ZEB1 and VEGF in villous tissues and serum would abnormally decrease,while the expression of miR-200 would be abnormally increased.Besides,the expression levels of ZEB1,miR-200 and VEGF have a direct correlation with RSA.There are correlations among the expression levels of ZEB1,miR-200 and VEGF,and joint diagnosis can be used to predict the occurrence of RSA.
