摘要
目的探讨基底动脉血管管径和颅神经疾病的相关性,分析相关危险因素及其临床意义。方法选取颅神经疾病患者76例为研究对象,同期以100例无颅神经疾病者做为对照。所有病例均行头颅三维磁共振血管成像,用3T MRI测量中脑水平T_(2)相斜坡中段基底动脉管径的大小。将研究对象按年龄分组为21~40、41~60、61~80岁组。颅神经疾病组内,包括三叉神经痛34例,面肌痉挛42例。记录并比较各组高血压、性别、体重指数、基底动脉管径大小等情况。结果颅神经疾病组的基底动脉管径明显高于对照组。颅神经疾病组管径平均值为(3.602±0.482)mm,对照组为(3.219±0.4900)mm(P<0.05)。各年龄组基底动脉管径平均值分别为2.419(0.4900);3.477(0.4022);4.288(0.5648),组间差异显著(P<0.05)。而三叉神经痛组和面肌痉挛组组间差异不明显(P>0.05),两者分别为3.677(0.4022)和3.583(0.5648)(P>0.05)。颅神经疾病的所选危险因素中高血压、基底动脉管径相关性最大。Logistic回归分析结果显示基底动脉管径(OR=4.523)、高血压(OR=2.273),是颅神经疾病的危险因素(P<0.05)。结论(1)人群中基底动脉管径、高血压是颅神经疾病的危险因素;(2)基底动脉管径随年龄增长。
Objective To explore the correlation between the diameter of basilar artery and cranial nerve diseases,and to analyze the related risk factors and their clinical significance.Methods 76 patients with cranial nerve diseases were selected as research objects,and 100 patients without cranial nerve diseases as control.Three dimensional magnetic resonance angiography(MRA)was performed in all cases,and the diameter of basilar artery on T_(2)image of middle clival brainstem level was measured by 3T MRI.The subjects were divided into 21-40,41-60 and 61-80 age groups according to their ages.In the cranial nerve disease group,there were 34 patients with trigeminal neuralgia and 42 patients with hemifacial spasm.The hypertension,sex,BMI(body mass index)and diameter of basilar artery were recorded and compared among the groups.Results The diameter of basilar artery in cranial nerve disease group was significantly higher than that in the control group.The average diameter of cranial nerve disease group was 3.602mm±0.482,and that of control group was 3.219±0.4900(P<0.05).The average diameters of basilar artery in each age group were 2.419(0.4900).3.477(0.4022)and 4.288(0.5648),with significant differences between groups(P<0.05).However,there was no significant difference between trigeminal neuralgia group and hemifacial spasm group(P>0.05),and they were 3.677(0.4022)and3.583(0.5648),respectively(P>0.05).Among the selected risk factors of cranial nerve diseases,hypertension and basilar artery diameter had the greatest correlation.Logistic regression analysis showed that basilar artery diameter(OR=4.523)and hypertension(OR=2.273)were risk factors closely related to cranial nerve diseases(P<0.05).Conclusion(1)Basilar artery diameter and hypertension are risk factors related to cranial nerve diseases.(2)The diameter of basilar artery increases with age.
