小剂量牛肺表面活性剂联合nCPAP在孕周<34周早产儿中的应用效果及对肺换气状态的影响

摘要 目的:探究小剂量牛肺表面活性剂联合鼻塞式持续气道正压通气(nCPAP)在孕周<34周早产儿中的应用效果及对肺换气状态的影响。方法:将2019年2月-2022年1月景德镇市第二人民医院的80例孕周<34周早产儿根据随机数字表法分为对照组40例和观察组40例。对照组采用nCPAP进行治疗,观察组则采用小剂量牛肺表面活性剂联合nCPAP进行治疗。比较两组的治疗总有效率、并发症发生率、治疗前后的血气分析指标(PaO_(2)及PaCO_(2))、氧合指数(OI)、脑氧饱和度(ScO_(2))及肺换气指标[最大吸气压(P_(imax))、最大呼气压(P_(emax))及肺泡-动脉氧分压差(P_(A-a)DO_(2))]。结果:治疗72 h后,观察组的治疗总有效率显著高于对照组,并发症发生率显著低于对照组,差异均有统计学意义(P<0.05),治疗前,两组的血气分析指标、OI、ScO_(2)及肺换气指标比较,差异均无统计学意义(P>0.05),治疗2、12及24 h后,观察组的血气分析指标、OI、ScO_(2)及肺换气指标均显著优于对照组,差异均有统计学意义(P<0.05)。结论:小剂量牛肺表面活性剂联合nCPAP在孕周<34周早产儿中的应用效果较好,且可显著改善早产儿的肺换气状态,在孕周<34周早产儿中的应用价值较高。 Objective:To investigate the application effect of low dose Calf Pulmonary Surfactant combined with nasal-continuous positive airway pressure(nCPAP)in preterm infants with gestational age<34 weeks and influence of pulmonary gas exchange state.Method:A total of 80 preterm infants with gestational age<34 weeks in the Second People’s Hospital of Jingdezhen from February 2019 to January 2022 were divided into control group 40 cases and observation group 40 cases by the method of random number table.The control group was treated with nCPAP,the observation group was treated with low dose Calf Pulmonary Surfactant combined with nCPAP.Then the total effective rates,complication rates,blood gas analysis indexes(PaO_(2),PaCO_(2) and pH),oxygenation index(OI),cerebral oxygen saturation(ScO_(2))and pulmonary gas exchange indexes[maximum inspiratory pressure(P_(imax)),maximum expiratroy pressure(P_(emax)),alveolarartelial oxygen tension difference(P_(A-a)DO_(2))before and after treatment of two groups were compared.Result:The total effective rate of observation group was significantly higher than that of control group at 72 hours after treatment,the total complication rate was significantly lower than that of control group,the differences were statistically significant(P<0.05).The blood gas analysis indexes,OI,ScO_(2) and pulmonary gas exchange indexes of two groups before treatment were no significant differences(P>0.05),the blood gas analysis indexes,OI,ScO_(2) and pulmonary gas exchange indexes of observation group at 2,12 and 24 hours after treatment were significantly better than those of control group,the differences were statistically significant(P<0.05).Conclusion:The application effect of low dose Calf Pulmonary Surfactant combined with nCPAP in preterm infants with gestational age<34 weeks is better,and it can significantly improve the pulmonary gas exchange state,so its application value in the preterm infants with gestational age<34 weeks is higher.
