摘要
目的评价快速康复护理在行经鼻垂体瘤切除术患者中的应用效果。方法回顾性分析2020年1月至2021年12月于华中科技大学同济医学院附属同济医院行经鼻垂体瘤切除术的104例患者资料,按护理方法的不同划分为对照组(54例)和观察组(50例)。对照组开展常规护理,观察组开展快速康复护理。比较2组的应激指标水平、并发症发生率、术后恢复情况及激素水平变化。结果术前12h和术后1d,2组的皮质醇、去甲肾上腺素水平均高于入院时,但观察组均较同期对照组更低,均存在统计学差异(P均<0.05)。观察组的呼吸困难等并发症总发生率与对照组相比更低(P<0.05)。与对照组比较,观察组的首次下床活动时间、肠鸣音恢复时间、首次排气时间均更早(P均<0.05),住院时间更短(P<0.05)。术后1d和出院时,2组的血清游离甲状腺素水平均升高,促甲状腺激素水平均降低,且观察组的上述激素水平均较同期对照组更优,比较均有统计学差异(P均<0.05)。结论将快速康复护理应用于行经鼻垂体瘤切除术患者中,可起到减轻应激反应、降低并发症发生率、加快术后恢复进程的积极效果,且有助于促进其术后甲状腺激素水平的恢复。
Objective To evaluate the application effect of fast rehabilitation nursing in the patients undergoing transnasal resection of pituitary tumor.Methods The data of 104 patients who underwent transnasal resection of pituitary tumor at Tongji Hospital,Tongji Medical College,Huazhong University of Science and Technology from January 2020 to December 2021 were retrospectively analyzed.They were divided into a control group(54 cases)and an observation group(50 cases)according to the different nursing methods.The control group carried out routine nursing,and the observation group carried out rapid rehabilitation nursing.The levels of stress indicators,incidence of complications,postoperative recovery,and changes in hormone levels were compared between the two groups.Results At 12 hr before surgery and 1 day after surgery,the levels of cortisol and norepinephrine in the two groups were higher than those at admission,but the levels in the observation group were lower than those in the control group(all P<0.05).The total incidence of complications such as dyspnea in the observation group was lower than that in the control group(P<0.05).Compared with the control group,the time to first ambulation,time to bowel sound recovery,time to first exhaust,and hospitalization time in the observation group were shorter(all P<0.05).At 1 day after surgery and at discharge,the serum free thyroxine level in the two groups was increased,and the thyroid-stimulating hormone level was decreased(all P<0.05).The above hormone levels in the observation group were better than those in the control group(all P<0.05).Conclusion The application of fast rehabilitation nursing in the patients undergoing transnasal resection of pituitary tumor can reduce the stress response and incidence of complications,speed up the postoperative recovery process,and help to promote the recovery of postoperative hormone levels.
