摘要
目的:探讨尿液8-OHdG、血清IL-6与糖尿病肾脏病相关性。方法:收集2017年09月—2019年05月就诊于东直门医院糖尿病肾脏病患者123例,收集受试者人口学信息、一般临床资料,进行肾功能、24 h尿蛋白定量等相关检查。依据DKD诊断、CKD分期、Mongensen分期等标准,将糖尿病肾脏病患者分为微量白蛋白尿期、大量蛋白尿期、肾衰竭早期,留取标本,检测尿液8-OHdG、血清IL-6水平。结果:微量白蛋白尿期、大量蛋白尿期、肾衰竭早期患者尿液8-OHdG(P=0.000)、血清IL-6(P=0.000)水平依次升高,差异有统计学意义。采用Spearman相关分析,尿液8-OHdG与24 h UTP呈高度正相关性(r=0.748,P=0.000),与eGFR呈中度负相关性(r=-0.502,P=0.000);血清IL-6水平与24 h UTP呈低度正相关性(r=0.397,P=0.000),与eGFR(CKD-EPI公式)呈低度负相关性(r=-0.330,P=0.000)。结论:尿液8-OHdG与DKD中高度相关,血清IL-6与DKD低度相关性,尿液8-OHdG、血清IL-6代表的氧化应激、炎症为2型糖尿病肾脏疾病的影响因素,尿液8-OHdG有可能成为反应糖尿病肾脏病病情进展及疗效评价的生物学标志物,且有望成为诊疗糖尿病肾脏病潜在的靶标。
Objective:To investigate the correlation between urine 8-OHDG and serum IL-6 and diabetic kidney disease.Methods:A total of 123 patients with diabetic kidney disease who were admitted to Dongzhimen Hospital from September 2017 to May 2019 were collected.Demographic information and general clinical data of the subjects were collected.Renal function,24-hour urinary protein quantification and other related tests were performed.According to the criteria of DKD diagnosis、CKD stage、Mogensen stage,the patients were divided into Microalbuminuria stage,large amount of albuminuria stage,early renal insufficiency according to the progression of diabetic kidney disease.The urine 8-OHdG and serum IL-6 levels were detected.Results:Urine 8-OHdG(P=0.000)and serum IL-6(P=0.000)levels increased successively in early,middle and late patients,and the differences were statistically significant.Using Spearman correlation analysis,urine 8-OHdG had a high positive correlation with 24 UTP(r=0.748,P=0.000),and a moderate negative correlation with EGFR(r=-0.502,P=0.000).Serum IL-6 level had a low positive correlation with 24 UTP(r=0.397,P=0.000)and a low negative correlation with EGFR(r=-0.330,P=0.000).Conclusion:Urine 8-OHDG is highly correlated with DKD,while serum IL-6 is low correlated with the process of DKD.Oxidative stress and inflammation represented by urine 8-OHDG and serum IL-6 are influencing factors of kidney disease progression in type 2 diabetes mellitus.Urine 8-OHdG May be a biomarker to reflect the progression and efficacy evaluation of diabetic kidney disease,and is expected to become a potential target for the diagnosis and treatment of diabetic kidney disease.
