摘要
目的比较盐酸羟考酮与枸橼酸舒芬太尼应用于腹腔热灌注化疗中的临床效果。方法选择2017年3月-2018年9月在本院拟在静脉全麻下行腹腔热灌注治疗的卵巢癌患者34例,采用随机数字表法分为盐酸羟考酮组(O组)和枸橼酸舒芬太尼组(S组),每组各17例。O组热灌注前10min静脉注射0.1mg/kg羟考酮,S组热灌注前10min静脉注射舒芬太尼0.1μg/kg,两组分别静脉给予0.02mg/kg咪达唑仑,丙泊酚起始计量0.4μg/kg·min靶控治疗,术中维持脑电双频指数BIS值40~60、手术体积描记指数SPI指数30~50之间。比较两组患者术前10min(T1)、术中25min(T2)、手术结束时(T3)患者的平均动脉压(MAP)、心率(HR)、血氧饱和度(SPO_(2))以及术中体动次数、体温波动值、呼吸抑制发生率、丙泊酚用药总量级麻醉苏醒时间。结果O组T1-T3的MAP、HR、SPO_(2)、术中体温波动值、麻醉苏醒时间与S组相比无统计学意义(P>0.05)。O组术中的体动次数、呼吸抑制发生率、丙泊酚总用量显著少于S组(P<0.05)。结论在腹腔热灌注化疗中应用盐酸羟考酮与枸橼酸舒芬太尼均可顺利完成手术,但应用盐酸羟考酮患者更为舒适有效,值得在临床推广使用。
Objective To compare the clinical effects of oxycodone hydrochloride and sufentanil citrate in intraperitoneal thermal perfusion chemotherapy.Methods 34 patients with ovarian cancer who were to be treated by intraperitoneal thermal perfusion under intravenous general anesthesia in our hospital from March 2017 to September 2018 were selected.They were randomly divided into oxycodone hydrochloride group(group O)and sufentanil citrate group(Group S),with 17 cases in each group.In group O,0.1mg/kg oxycodone was injected intravenously 10 minutes before thermal perfusion.In group S,sufentanil 0.1μg/kg was injected intravenously 10 minutes before thermal perfusion.Both groups were given 0.02mg/kg midazolam intravenously,and the initial dose of propofol was 0.4μg/kg·min.The bispectral index BIS value was maintained between 40~60 and the operation plethysmography index SPI was maintained between 30~50.The mean arterial pressure(MAP),heart rate(HR),blood oxygen saturation(SPO_(2)),number of intraoperative body movements,fluctuation of body temperature,incidence of respiratory depression,total anesthesia awakening time of propofol were compared between the two groups 10 minutes before operation(T1),25 minutes during operation(T2),and at the end of operation(T3).Results The MAP,HR,SPO_(2),intraoperative temperature fluctuation and anesthesia awakening time of T1~T3 in group O were not statistically significant compared with those in group S(P>0.05).The number of body movements,the incidence of respiratory depression and the total dosage of propofol in group O were significantly lower than those in group S(P<0.05).Conclusion Oxycodone hydrochloride and sufentanil citrate can successfully complete the operation in intraperitoneal thermal perfusion chemotherapy,but oxycodone hydrochloride is more comfortable and effective in patients,which is worthy of clinical application.
