摘要
为研究微创经皮椎弓根内固定术(minimally invasive percutaneous pedicle screws osteosynthesis, MIPPSO)对胸腰椎骨折患者功能障碍指数、伤椎高度以及疼痛程度的影响,回顾性分析2018年10月—2020年10月期间进行微创经皮椎弓根内固定术治疗的53例胸腰椎骨折患者临床资料,观察记录所有手术患者创面愈合时间、手术时间、术中出血量;比较患者术前、术后的功能障碍指数(Oswestry disability index, ODI)、疼痛程度(visual analogue scale, VAS)以及伤椎高度与Cobb角。结果表明:53例患者胸腰椎手术均已完成。其中创面愈合时间为(7.34±1.23) d,手术时间为(83.98±6.22) min,术中出血量为(66.45±5.11) mL。术后6 d与术后10个月,ODI、VAS评分均明显低于术前(P<0.05)。术后6 d与术后10个月,患者伤椎前缘高度均明显高于术前(P<0.05),但Cobb角度明显均低于术前(P<0.05)。胸腰椎骨折患者进行经皮椎弓根内固定术治疗,能够改善患者腰椎功能障碍,同时减少术中出血量,缩短手术时间,促进创面快速愈合,并有效减轻术后机体疼痛,值得临床应该与推广。
To study the effect of minimally invasive percutaneous pedicle screws osteosynthesis on the dysfunction index, injury height and pain degree of thoracolumbar fracture patients, the clinical data of 53 cases with thoracolumbar fractures treated by minimally invasive percutaneous pedicle internal fixation from October 2018 to October 2020 were analyzed retrospectively. The wound healing time, operation time, intraoperative bleeding volume were observed. The preoperative and postoperative disability index(ODI), degree of pain(VAS), the height of injured vertebra and Cobb angle were compared. The results show that 53 cases of thoracolumbar surgery have been completed. The wound healing time is(7.34±1.23) d, the operation time is(83.98±6.22) min, the intraoperative bleeding volume is(66.45±5.11) mL. The scores of ODI and VAS at 6 days and 10 months after operation are significantly lower than those before operation(P<0.05). At 6 days and 10 months after operation, the height of the anterior edge of the injured vertebrae is significantly higher than that before operation(P<0.05), but the Cobb angle is significantly lower than that before operation(P<0.05). The treatment of thoracolumbar fracture patients with percutaneous pedicle internal fixation can improve the lumbar dysfunction, reduce the amount of bleeding during the operation, shorten the operation time, promote the rapid healing of the wound, and effectively reduce the postoperative body pain, which is worthy of clinical application and promotion.
