摘要
目的 研究血清核因子E2相关因子(Nrf2)、胎盘生长因子(PLGF)在妊娠期糖尿病(GDM)患者中的水平变化及其与胰岛素抵抗的关系。方法 选取南京医科大学附属淮安第一医院2019年3月至2021年9月收治的103例GDM患者为研究组,并选择同期的100例正常孕妇为对照组,检测两组孕妇的血清Nrf2、PLGF水平,以及胰岛素抵抗指标[胰岛素抵抗指数(HOMA-IR)、胰岛β细胞功能指数(HOMA-β)]。比较GDM患者和正常孕妇血清Nrf2、PLGF水平及HOMA-IR、HOMA-β,采用Pearson相关法分析血清Nrf2、PLGF水平与HOMA-IR、HOMA-β的相关性,采用多元线性逐步回归模型分析HOMA-IR、HOMA-β的影响因素。结果 GDM组患者血清Nrf2、PLGF水平显著低于正常孕妇组(t值分别为4.558、27.960,P<0.01)。GDM组患者HOMA-IR显著高于正常孕妇组(t=10.531,P<0.01),而HOMA-β显著低于正常孕妇(t=14.501,P<0.01)。Pearson相关性分析显示,GDM患者血清Nrf2、PLGF水平与HOMA-IR呈显著负相关(r值分别为-0.541、-0.486,P<0.05),与HOMA-β呈显著正相关(r值分别为0.612、0.574,P<0.05)。多元线性逐步回归模型分析显示,孕期体重增加值、Nrf2、PLGF是HOMA-IR的显著影响因素(t值分别为7.485、12.596、13.174,P<0.01),HbAlc、Nrf2、PLGF是HOMA-β的显著影响因素(t值分别为9.987、10.526、12.485,P<0.01)。结论 GDM患者血清Nrf2、PLGF水平显著降低,且与胰岛素抵抗呈显著负相关。
Objective To investigate the changes of serum nuclear factor E2 related factor(Nrf2) and placental growth factor(PLGF) in patients with gestational diabetes mellitus(GDM) and their relationship with insulin resistance.Methods 103 women with GDM in First Hospital Affiliated of Huaian of Nanjing Medical University from March 2019 to September 2021 were selected as study group, and 100 normal pregnant women in the same period were selected as control group.The serum Nrf2 and PLGF levels were detected, and homeostasis model assessment insulin resistance index(HOMA-IR) and β cell function index(HOMA-β) were also detected.The levels of serum Nrf2,PLGF,HOMA-IR and HOMA-β between women with GDM and normal group were compared.Pearson correlation was used to analyze the correlation between serum Nrf2 and PLGF levels, HOMA-IR and HOMA-β,and multiple linear stepwise regression model was used to analyze the factors affecting HOMA-IR and HOMA-β.Results The Nrf2 and PLGF levels in GDM group were significantly lower than those in normal group(t=4.558 and 27.960,respectively, P<0.01).The HOMA-IR in GDM group was significantly higher than that in normal pregnant women group(t=10.531,P<0.01),and HOMA-β was significantly lower than that in normal pregnant women group(t=14.501,P<0.01).Pearson correlation analysis showed that serum Nrf2 and PLGF levels were significantly negatively correlated with HOMA-IR(r=-0.541 and-0.486,respectively, P<0.05),and serum Nrf2 and PLGF levels were significantly positively correlated with HOMA-β(r=0.612 and 0.574,respectively, P<0.05).Multiple linear stepwise regression model analysis showed that weight gain during pregnancy, Nrf2 and PLGF were significant influencing factors of HOMA-IR(t=7.485,12.596 and 13.174,respectively, P<0.01).HbAlc, Nrf2 and PLGF were the significant influencing factors of HOMA-β(t=9.987,10.526 and 12.485,respectively, P<0.01).Conclusion Serum Nrf2 and PLGF levels were significantly decreased in GDM patients, which were negatively correlated with insulin resistance.
