摘要
目的了解东莞地区痤疮患者痤疮丙酸杆菌对大环内酯-林可酰胺类耐药的基因分布情况,探讨其耐药机制。方法分离、培养、鉴定痤疮丙酸杆菌后经药敏实验选取大环内酯类-林可酰胺类抗生素耐药菌株53株(单一耐药13株,多重耐药40株)和敏感菌株4株,进行全基因组鸟枪法重测序,与痤疮丙酸杆菌标准株ATCC 6919进行对照,分析本地区痤疮丙酸杆菌中耐药基因情况。结果敏感菌株均无23S rRNA基因点突变或携带ermX耐药基因;单一耐药(克拉霉素或阿奇霉素)均无携带ermX耐药基因,但3株对克拉霉素低、中度耐药的菌株在23S rRNA基因287、555、287\514位点存在突变;40株多重耐药菌株中,12株23S rRNA基因发生2058点突变(10株由A→G突变,2株由A→T突变),26株携带ermX耐药基因,且携带ermX耐药基因与发生2058点突变的菌株不存在交叉重叠,剩余2株未发现点突变或携带ermX耐药基因。结论东莞地区痤疮患者痤疮丙酸杆菌对大环内酯-林可酰胺类多重耐药的主要基因型为携带ermX耐药基因(65%)和23S rRNA基因2058点突变(30%)。此外,观察到23S rRNA其他位点突变,不排除与耐药相关,如287点突变可能与克拉霉素耐药有关。
Objective To understand the distribution of Macrolide-Lincosamides resistance gene of Corynebacterium acnes in acne patients in Dongguan area,and to predict the resistance mechanism of Macrolide-Lincosamides.Methods Strains of Corynebacterium acnes were isolated from acne patients,then cultured,identified and tested for drug susceptibility in vitro using agar dilution method.Based on drug susceptibility test,57 strains with Macrolide-Lincosamide antibiotic-resistant or sensitive ones were obtained,which contained 13 single-drug resistance strains,40 multi-drug resistance strains and 4 sensitive strains.All 57 strains were subjected to high-throughput resequencing using the whole gene shotgun method,and then compared to the standard Corynebacterium acnes strain ATCC 6919 to analyze the resistance genes in Corynebacterium acnes in this area.Results All 4 sensitive strains of Corynebacterium acnes didn′t have 23 S rRNA gene point mutations or ermX resistance gene;13 single-drug resistance strains of Corynebacterium acnes which were only resistant to clarithromycin or azithromycin didn′t carry the ermX resistance gene,but 3 strains with low-to-moderate resistance to clarithromycin had mutation in 287,555,287\514 site of the 23 S rRNA gene,respectively;Among the 40 multi-drug resistant strains of Corynebacterium acnes,10 strains were found to be A→G mutations at the 2058 site of the 23 S rRNA gene,2 strains were found to be A→T mutations at the 2058 site of the 23 S rRNA gene.In addition,26 strains were found to carry the ermX resistance gene,and the remaining 2 highly multi-drug resistant bacteria were found neither point mutations nor carrying ermX resistance genes.There was no cross-over overlap between strains carrying the ermX resistance gene and mutations at the 2058 site of the 23 S rRNA gene.Conclusion Carrying ermX resistance gene(65%)and 23 S rRNA gene 2058 site mutation(30%)are the main genotypes of Corynebacterium acnes resistance to Macrolide-Lincosamide in acne patients in Dongguan area.In addition,several mutations sites in 23 S rRNA were observed which might be related to drug resistance.For example,287 point mutation may be related to clarithromycin resistance.
