摘要
龋病是口腔常见病、多发病,充填修复是主要的治疗手段。光固化复合树脂由于出色的美学性能、良好的力学性能、操作简便以及易修理等优点,现已成为牙科最广泛应用的充填修复材料。但市售复合树脂自身多无抗菌性,且该类材料较银汞合金等其他牙齿充填材料表面易积聚菌斑生物膜。菌斑生物膜是龋病的始动因素,可发酵碳水化合物产酸,使充填体周围的牙体硬组织脱矿,形成继发龋。继发龋是树脂类材料修复失败的主要原因,目前针对其有效管理仍是全球性挑战。抗附着剂,如2-甲基丙烯酰氧乙基磷酸胆碱,可抑制树脂表面蛋白质和细菌附着,进而减少菌斑生物膜的形成。抗菌剂,如壳聚糖、纳米氧化锌,可抑制或杀死树脂表面的致龋菌。促矿化剂,如羟基磷灰石、硅酸铌,不仅可中和树脂修复体周围的酸性环境,且释放钙、磷等离子,可促进脱矿的牙体硬组织再矿化。本文旨在阐述近年来含有上述防龋潜力材料的复合树脂的研究情况,未来这些树脂有望减少口腔临床中继发龋的发生,延长修复体的寿命,从而促进口腔健康。
Dental caries is a common and frequently occurring disease in oral cavity,and filling restoration is the main treatment.Due to their excellent aesthetic properties,good mechanical properties,simple operation and easy repair,light cured resin composites have become the most widely used filling and restorative materials in dentistry.However,most of the commercially available resin composites have no antibacterial properties,and the surface of this class of material are more likely to accumulate plaque biofilm than other dental filling materials such as amalgam.Plaque biofilm is the initiating factor of caries,which can ferment carbohydrate to produce acid,demineralize the dental hard tissue around the restoration,and form secondary caries.Secondary caries is the main reason for the failure of resin-based restoration,and its effective management is currently a global challenge.Anti-adhesion agents,such as 2-methacryloyloxyethyl phosphorylcholine,can reduce the adhesion of protein and bacteria to the composite surface,thereby decreasing the formation of plaque biofilm.Antibacterial agents,such as chitosan and zinc oxide nanoparticles,can inhibit or kill cariogenic bacteria on the composite surface.Mineralizers,such as hydroxyapatite and niobium silicate,cannot only neutralize the acidic environment around the resin restoration,but also release calcium,phosphorus and other ions,which can promote the remineralization of demineralized tooth hard tissue.This paper aims to elaborate the research status of resin composites containing the above-mentioned anti-caries potential materials in recent years.In the future,these composites are expected to reduce the occurrence of clinical secondary caries in the oral cavity and prolong restoration life,thereby promoting oral health.
