摘要
目的 探讨头皮针联合悬吊训练对痉挛型脑瘫患儿肢体痉挛及粗大运动功能的影响。方法 选取2018年6月至2021年6月100例痉挛型脑瘫患儿,采用随机数字表法分为研究组48例与对照组47例。两组均采用常规康复治疗,在此基础上对照组给予悬吊训练,6次/周;研究组在对照组基础上联合头皮针治疗,1次/天,6次/周,两组均连续治疗6个月。比较两组治疗前后疗效、下肢临床痉挛指数(CSI)、10 min步行速度(10 mMWS)、Berg平衡量表(BBS)、上下肢简式Fugl Meyer运动功能量表(FMA)、粗大运动功能评定量表(GMFM)-88评分变化,以及不良反应发生情况。结果 研究组治疗总有效率高于对照组(P<0.05);治疗后,两组CSI低于治疗前,10 mMWS、BBS、FMA评分及GMFM-D、GMFM-E评分均高于治疗前,且研究组优于对照组,差异有统计学意义(P<0.05)。两组治疗期间均未发生明显不良反应。结论 两种治疗方法对于痉挛型脑瘫患儿肢体痉挛及粗大运动功能改善均有效,但结合头皮针联合悬吊训练治疗的总体疗效要优于单独悬吊治疗。
Objective To investigate the effect of scalp acupuncture combined with suspension training on the improvement of limb spasticity and gross motor function in children with spastic cerebral palsy(SCP).Methods One hundred children with SCP from June 2018 to June 2021 were selected. The sick children were divided into a control group(n=47) and a study group(n=48) using the random number table method. Both groups were treated with conventional rehabilitation therapy. On this basis, the control group was given suspension training, 6 times/week. The study group was treated with suspension training combined with scalp acupuncture(1 time/day, and 6 times/week). Both groups were continuously treated for 6 months. The lower limb spasticity estimated by Clinical Spasticity Index(CSI), 10 min walking speed(10 mMWS), balance function estimated by Berg Balance Scale(BBS), motor function estimated by upper and lower limb simple Fugl Meyer Motor Function Scale(FMA), and gross motor function estimated by Gross Motor Function Rating Scale(GMFM)-88 were compared before and after treatment between the two groups. The occurrence of adverse effects was also compared.Results The total effective rate of treatment was higher in the study group than that in the control group(P<0.05). After treatment, CSI was lower and 10 mMWS, BBS, FMA scores and GMFM-D and GMFM-E scores were higher than those before treatment in both groups and the study group was better than the control group(P<0.05). No significant adverse reactions occurred during treatment in either group.Conclusion Both treatments are effective in improving limb spasticity and gross motor function in children with SCP, but the overall efficacy of scalp acupuncture combined with suspension training treatment is better than that of suspension treatment alone.
