ICU患者呼吸机相关肺炎耐药菌感染发生的影响因素及护理对策 被引量:8

Influencing Factors and Nursing Countermeasures of Ventilator-associated Pneumonia Drug-resistant Bacteria Infection in ICU Patients

摘要 目的:探究在ICU患者中引发感染呼吸机相关肺炎耐药菌的相关影响因素,并提出相应的护理对策。方法:采用横断面调查设计,选取本院2021年1月—2022年5月院内使用呼吸机的ICU患者62例为研究对象,分析呼吸机相关肺炎耐药菌的发病率,并对耐药率进行检测分析;根据判断是否感染相关肺炎耐药菌分组为观察组(相关肺炎耐药菌感染)和对照组(相关肺炎耐药菌未感染),对其相关影响因素进行Logistic回归分析。结果:62例患者中肺炎克雷伯杆菌、鲍曼不动杆菌、金黄色葡萄球菌、流血嗜血杆菌、大肠埃希菌发病率/耐药率分别为20.97%/38.46%、16.13%/30.00%、12.90%/25.00%、9.68%/50.00%、4.84%/33.33%。ICU住院时间、通气时间、气管插管操作、合并患有呼吸系统疾病是感染呼吸机相关肺炎耐药菌的4个独立危险因素(P<0.05)。结论:ICU患者感染呼吸机相关肺炎耐药菌与合并患有呼吸系统疾病、气管插管操作、ICU住院时间、通气时间等密切相关,临床应通过采取对应护理措施,降低感染率。 Objective:To explore the related influencing factors of ventilator-associated pneumonia resistant bacteria in ICU patients,and put forward the corresponding nursing countermeasures.Methods:A cross-sectional survey design was used to select 62 ICU patients who used ventilator in our hospital from January 2021 to May 2022.The incidence of ventilatorassociated pneumonia resistant bacteria was analyzed,and the resistance rate was detected and analyzed.The patients were divided into observation group(related pneumonia resistant bacteria infection)and control group(related pneumonia resistant bacteria infection)according to whether they were infected or not.Logistic regression analysis was performed on the related influencing factors.Results:The incidence/drug resistance rates of Klebsiella pneumoniae,Acinetobacter baumannii,Staphylococcus aureus,Haemophilus haemophilus and Escherichia coli in 62 patients were 20.97%/38.46%,16.13%/30.00%,12.90%/25.00%,9.68%/50.00%and 4.84%/33.33%,respectively.ICU hospitalization time,ventilation time,tracheal intubation operation,and respiratory diseases were the four independent risk factors for the infection of ventilator-associated pneumonia resistant bacteria(P<0.05).Conclusion:The infection of ventilator-associated pneumonia resistant bacteria in ICU patients is closely related to respiratory diseases,tracheal intubation operation,ICU hospitalization time and ventilation time.Clinical nursing measures should be taken to reduce the infection rate.
