摘要 目的探究迭代重建算法在冠状动脉支架CT血管成像中的应用研究。方法选取2016年12月至2018年12月在我院行冠状动脉检查的冠心病患者(支架置入术后评估)共100例,在120kV管电压下分别采用传统滤波反投影法(FBP)及迭代重建算法(siemens definition flash DSCT双源CT机,SAFIRE)进行图像重建,对比分析两组图像噪声(SD)、信号噪声比(SNR)、对比噪声比(CNR)以及双源CT在不同心率、支架直径、支架分布下的敏感性、特异性、阳性预测值和阴性预测值。结果IR图像质量明显高于FPB图像(P<0.05);IR图像的SD明显低于FBP图像,且SNR、CNR明显高于FBP图像(P<0.05);同时,多层螺旋CT对不同支架直径的冠状动脉狭窄诊断有明显差异性(P<0.05)而对不同心率、支架分布的诊断无明显差异(P>0.05)。结论迭代重建算法在冠状动脉支架血管成像中能明显提高图像质量,降低图像噪声并显著提高信噪比与对比噪声比,能够准确诊断冠状动脉的狭窄病变,值得临床上大力推广应用。 Objective to explore the application of iterative reconstruction algorithm in CT angiography of Coronary stent.Methods a total of 100 patients with coronary heart disease in our hospital from December 2016 to December 2018 were selected,under 120 kv tube voltage applied traditional filter back projection place(FBP)and iterative reconstruction algorithm(Siemens definition Flash DSCT,SAFIRE)image reconstruction,observing the image quality of two groups of patients and subjective rating,compared two groups of image noise(SD),signal to noise ratio(SNR),contrast to noise ratio(CNR)and dual-source CT under different heart rate,stents,the diameter distribution of sensitivity,specificity,positive predictive value and negative predictive value.Results the image quality of the IR image was significantly higher than that of the FBP image(P<0.05).The SD of the IR image was significantly lower than the FBP image,and SNR and CNR were significantly higher than the FBP image(P<0.05).At the same time,the diagnosis of coronary artery stenosis with different stent diameters was significantly different(P<0.05),and there was no significant difference in the diagnosis of different heart rate and stent distribution(P>0.05).Conclusion iterative reconstruction algorithm can significantly improve the quality of images,and significantly improve the signal-to-noise ratio and reduce image noise contrast to noise ratio,can accurate diagnosis of stenosis of coronary artery lesions,is worth promoting clinical application.
