摘要
目的 分析左甲状腺素钠片治疗妊娠伴甲状腺功能减退效果。方法 选取2018年2月-2019年9月医院收治的妊娠伴甲状腺功能减退患者120例,依照入院时间先后顺序分为对照组60例为基础性治疗,观察组60例接受左甲状腺素钠片治疗,对比患者治疗后临床指标变化和治疗总有效率。结果 两组患者治疗前游离甲状腺素(free thyroxine,FT_(4))及游离三碘甲状腺原氨酸(free triiodothyronine,FT3)等指标比较,差异无统计学意义(P>0.05);治疗后观察组患者促甲状腺激素(thyroid stimulating hormone,TSH)及FT3指标优于对照组,差异有统计学意义(P<0.05);治疗前两组患者血脂水平比较,差异无统计学意义(P>0.05);治疗后观察组患者血脂指标优于对照组,差异有统计学意义(P<0.05);治疗前两组患者一氧化氮(nitric oxide,NO)、内皮素-1(endothelin 1,ET-1)以及内皮依赖性扩张百分率(flow-mediated dilatation percentage,FMD%)比较,差异无统计学意义(P>0.05);治疗后观察组患者FMD%、NO、ET-1比较,差异有统计学意义(P<0.05);观察组患者不良妊娠率低于对照组,差异有统计学意义(P<0.05)。结论 为妊娠伴甲状腺功能减退患者采用左甲状腺素钠片进行治疗时不仅可提升患者疾病治疗效果,同时也可改善甲状腺功能,孕妇血管内皮功能,调节血脂水平,有临床应用价值。
Objective To analyze the effect of levothyroxine sodium tablets in the treatment of pregnancy with hypothyroidism.Methods A total of 120 pregnant patients with hypothyroidism who were admitted to our hospital from February 2018 to September 2019 were selected and divided into the control group with 60 cases as the basic treatment according to the order of admission time,and the observation group with 60 cases of levothyroxine sodium tablets,and compared the changes of clinical indicators and the total effective rate of treatment after treatment.Results There was no significant difference in free thyroxine(FT_(4)) and free triiodothyronine(FT3) between the two groups before treatment(P>0.05).After treatment,thyroid stimulating hormone(TSH) and FT3 indexes of the observation group were better than those of the control group(P<0.05).Before treatment,there was no significant difference in blood lipid level between the two groups(P>0.05).After treatment,the blood lipid index of the observation group was better than that of the control group(P<0.05).Before treatment,there was no significant difference in nitric oxide(NO),endothelin 1(ET-1) and flow-mediated dilatation percentage(FMD%) between the two groups(P>0.05).After treatment,there was no significant difference in FMD%,NO and ET-1 in the observation group(P<0.05).The adverse pregnancy rate of the observation group was lower than that of the control group(P<0.05).Conclusion The use of levothyroxine sodium tablets for the treatment of pregnant patients with hypothyroidism can not only improve the treatment effect of the patient’s disease,endothelial function of pregnant women,but also improve thyroid function and regulate blood lipid levels,which has clinical application value.
