摘要
目的探讨黄芩汤灌肠疗法联合美沙拉嗪治疗溃疡性结肠炎的临床疗效及对炎性因子水平的影响。方法选取2020年1月—2021年1月牡丹江医学院附属第二医院收治的溃疡性结肠炎患者80例作为研究对象。采用随机数字表法将其分为观察组与对照组,对照组40例口服美沙拉嗪治疗,观察组40例则在美沙拉嗪治疗基础上配合黄芩汤灌肠治疗,对比两组溃疡性结肠炎的治疗效果及炎性因子水平。结果治疗后观察组溃疡性结肠炎腹泻、腹痛、脓血便症状积分明显低于对照组(P<0.05);观察组治疗总有效率明显高于对照组(P<0.05);治疗后观察组白介素-1β(interleukin-1β,IL-1β)、基质金属蛋白酶(matrix metalloproteinase-1,MMP-1)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、血沉(erythrocyte sedimentation rate,ESR)明显低于对照组(P<0.05);观察组不良反应发生率略低于对照组,但差异无统计学意义(P>0.05);随访调查结果显示观察组治疗后6个月复发率明显低于对照组(P<0.05)。结论采用黄芩汤灌肠联合美沙拉嗪治疗溃疡性结肠炎疗效显著,能够有效改善患者脓血便、腹痛、腹泻等症状体征以及血清炎症因子水平,降低溃疡性结肠炎复发风险,具有应用及推广价值。
Objective To explore the clinical efficacy of Huangqin decoction enema therapy combined with mesalazine in the treatment of ulcerative colitis and its influence on the level of inflammatory factors.Methods A total of 80 patients with ulcerative colitis admitted to The Second Affiliated Hospital of Mudanjiang Medical University from January 2020 to January 2021 were selected as the research subjects.They were divided into observation group and control group by random number table method,40 cases in the control group were treated with oral mesalazine,and 40 cases in the observation group were treated with Huangqin decoction enema on the basis of mesalamine treatment.The treatment effect and inflammatory factor levels of ulcerative colitis in two groups were compared.Results After treatment,the scores of symptoms of ulcerative colitis,diarrhea,abdominal pain,pus and blood in the observation group were significantly lower than those in the control group(P<0.05);the total effective rate of treatment in the observation group was significantly higher than that of the control group(P<0.05);after treatment,interleukin-1β(IL-1β),matrix metalloproteinase-1(MMP-1),tumor necrosis factor-α(TNF-α),erythrocyte sedimentation rate(ESR)and the incidence of adverse reactions in the the observation group were significantly lower than those in control group(P<0.05);the incidence of adverse reactions in the observation group was slightly lower than that in the control group,but the difference was not statistically significant(P>0.05);follow-up survey,the results showed that the recurrence rate of the observation group was significantly lower than that of the control group 6 months after treatment(P<0.05).Conclusion Huangqin decoction enema combined with mesalazine has a significant effect in the treatment of ulcerative colitis.It can effectively improve the symptoms and signs of patients with pus and blood in the stool,abdominal pain,diarrhea,and the level of serum inflammatory factors,and reduce the risk of ulcer recurrence.It has application and promotion value.
