摘要
目的:探讨断层超声显像对胎儿心脏大动脉畸形的诊断效能。方法:回顾性分析2016年1月—2020年12月期间于我院接受诊疗的200例疑似胎儿心脏大动脉畸形孕妇的临床资料,所有孕妇均接受断层超声显像检查,以胎儿出生后检查结果或病理尸检为对照,分析断层超声显像对胎儿心脏大动脉畸形的诊断价值。结果:断层超声显像诊断胎儿心脏大动脉畸形的灵敏度为96.33%、特异度为92.31%、准确度为94.50%、阳性预测值为93.75%、阴性预测值为95.45%;断层超声显像诊断胎儿心脏大动脉畸形类型与胎儿出生后检查、病理尸检结果具有极好一致性(Kappa=0.872)。结论:断层超声显像可清晰显示胎儿心脏大动脉畸形切面,利于判断心脏大动脉畸形类型,具有较高诊断效能,可为产后干预及临床处理提供依据。
Objective:To investigate the diagnostic performance of tomographic ultrasonography in fetal cardiac aortic malformation.Methods:The clinical data of 200 pregnant women with suspected fetal cardiac aortic malformation who were treated in our hospital from January 2016 to December 2020 were retrospectively analyzed.Pathological autopsy was used as a control to analyze the diagnostic value of tomographic ultrasonography in fetal cardiac aortic malformation.Results:The sensitivity,specificity,94.50%,93.75%,and 95.45%negative predictive value of tomographic ultrasonography in the diagnosis of fetal cardiac aortic malformation.The types of large arterial malformations were in excellent agreement with postnatal examination and pathological autopsy results(Kappa=0.872).Conclusion:Tomographic ultrasonography can clearly display the section of fetal cardiac aortic malformation,which is helpful for judging the type of cardiac aortic malformation,has high diagnostic efficiency,and can provide a basis for postpartum intervention and clinical treatment.
