彩色多普勒超声引导下细针穿刺活检在甲状腺瘤诊断中的应用价值 被引量:6

Application values of fine needle aspiration biopsy guided by color Doppler ultrasound for the diagnosis of thyroid tumors

摘要 目的 探讨彩色多普勒超声引导下细针穿刺活检在甲状腺瘤诊断中的应用价值。方法 选取2020年5月至2021年8月间延安大学咸阳医院收治的78例甲状腺瘤患者作为研究对象,根据随机数表法分为彩超组和对照组,每组39例。彩超组给予彩色多普勒超声引导下细针穿刺活检,对照组给予传统细针穿刺活检,判断诊断效果。结果 在彩超组中,组织病理诊断为甲状腺良性肿瘤17例,甲状腺恶性肿瘤22例;彩色多普勒超声引导下细针穿刺活检诊断为甲状腺良性肿瘤18例,甲状腺恶性肿瘤21例。在对照组中,组织病理诊断为甲状腺良性肿瘤16例,甲状腺恶性肿瘤23例,细针穿刺活检诊断为甲状腺良性肿瘤22例,甲状腺恶性肿瘤17例。甲状腺恶性肿瘤的彗星尾、边缘不规则、垂直位、微钙化等占比与甲状腺良性肿瘤对比,差异有统计学意义(P<0.05)。甲状腺瘤的内部回声特征对比,差异有统计学意义(P<0.05)。彩色多普勒超声引导下细针穿刺活检鉴别诊断甲状腺瘤的敏感性与特异性为100.0%和95.5%,传统细针穿刺活检鉴别诊断甲状腺瘤的敏感性与特异性为100.0%和73.9%。结论 彩色多普勒超声引导下细针穿刺活检在甲状腺瘤中应用,能提高诊断的特异性,具有方便、安全等优势,有利于预测恶性风险,有很好的临床应用价值。 Objective To explore the application value of fine needle aspiration biopsy guided by color Doppler ultrasound for the diagnosis of thyroid tumors. Methods From May 2020 to August 2021, a total of 78 patients with thyroid tumors were selected at Yan’an University Xianyang Hospital. They were divided into color Doppler ultrasound group and a control group with 39 patients each using random number table. Color Doppler ultrasound-guided fine-needle aspiration biopsy were performed in the color Doppler ultrasound group, and traditional fine needle aspiration biopsy were performed in the control group. The diagnostic efficacy were determined. Results In the color Doppler ultrasound group, 17 patients were diagnosed with benign thyroid tumors, and 22 patients were diagnosed with malignant thyroid tumors histopathologically, and 18 patients were diagnosed with benign thyroid tumors and 21 patients were diagnosed with malignant thyroid tumors by fine needle aspiration biopsy guided by color Doppler ultrasound. In the control group, 16 patients were diagnosed with benign thyroid tumors and 23 patients with malignant thyroid tumors by histopathology, and 22 patients were diagnosed with benign thyroid tumors and 17 patients with malignant thyroid tumors by fine needle aspiration biopsy. The proportions of comet tails, irregular edges, vertical positions, and microcalcifications of malignant thyroid tumors were significantly different from those of benign thyroid tumors all(P<0.05). There were sinificant differences in internal echo characteristics of thyroid tumors between the two groups(all P<0.05). The sensitivity and specificity of fine-needle aspiration biopsy under the guidance of ultrasound color Doppler ultrasound for the differential diagnosis of thyroid tumors were 100.0% and 95.5%, respectively and the sensitivity and specificity of traditional fine-needle aspiration biopsy for the differential diagnosis of thyroid tumors was 100.0% and 73.9%, respectively. Conclusion The application of color Doppler ultrasound-guided fine-needle aspiration biopsy can improve the diagnostic specificity for thyroid tumors. It has the advantages of convenience and safety, is conducive to predicting the risk of malignancy, and has good clinical application value.
