摘要
目的 探讨经腹膜外改良逆行膀胱全切术与膀胱部分切除术治疗肌层浸润性膀胱癌对氧化应激因子及预后的影响。方法 选取2015年12月至2020年10月在中国人民解放军联勤保障部队第九九〇医院诊治的80例肌层浸润性膀胱癌患者,根据手术方式分为经腹膜外改良逆行膀胱全切术(全切组)和膀胱部分切除术(部分切除组),每组40例。对比两组围手术期临床指标(留置导尿管时间、手术时间、术中出血量、住院时间)、术前和术后7 d氧化应激反应[血清谷胱甘肽过氧化物酶(GSG-Px)、超氧化物歧化酶(SOD)、丙二醛(MDA)]及预后(并发症、术后1 a复发率)情况。结果 部分切除组留置导尿管时间、手术时间、住院时间短于全切组,术中出血量少于全切组(P<0.05)。术后7 d,两组GSG-Px、SOD水平均较术前降低(P<0.05),两组MDA水平均较术前升高,且术后7 d部分切除组GSG-Px、SOD水平均较全切组高,部分切除组MDA水平均较全切组低(P<0.05)。部分切除组总不良反应率(17.50%)较全切组(37.50%)低(P<0.05),但两组术后1 a复发情况差异无统计学意义(P>0.05)。结论 经腹膜外改良逆行膀胱部分切除术相较于膀胱全切术,创伤小,有利于缩短手术及住院时间,减少术中出血量,缓解术后氧化应激反应,同时减少不良反应的发生。
Objective To explore the effect of modified extraperitoneal retrograde total cystectomy and partial cystectomy for myometrial invasive bladder cancer on oxidative stress factors and prognosis.Methods A total of 80 patients with myometrial invasive bladder cancer diagnosed and treated in the 990 th Hospital of the People’s Liberation Army Joint Logistic Support Force from December 2015 to October 2020 were selected.According to the surgical methods,the patients were divided into extraperitoneal modified retrograde total cystectomy(total resection group) and partial cystectomy(partial resection group),with 40 cases in each group.Perioperative clinical indicators(indwelling catheter time,operation time,intraoperative blood loss,hospitalization time),preoperative and postoperative 7 days oxidative stress response [serum glutathione peroxidase(GSG-Px),superoxide dismutase(SOD),malondialdehyde(MDA)] and prognosis(complications,recurrence rate in 1 year after surgery) were compared between the two groups.Results The time of indwelling catheter,operation time and hospital stay in the partial resection group were shorter than those in the total resection group,and intraoperative blood loss was less than that in the total resection group(P0.05).Conclusion Compared with total cystectomy,modified extraperitoneal retrograde partial cystectomy has less trauma,which is beneficial to shorten the operation and hospitalization time,reduce intraoperative blood loss,relieve postoperative oxidative stress,and reduce adverse reactions.
