摘要
目的 比较腹腔镜淋巴结清扫术与开腹淋巴结清扫术治疗卵巢癌的疗效及安全性。方法 依据治疗方式的不同将100例卵巢癌患者分为观察组和对照组,每组50例,观察组患者给予腹腔镜淋巴结清扫术治疗,对照组患者给予开腹淋巴结清扫术治疗。比较两组患者的手术相关指标、体力状况[卡氏功能状态(KPS)评分]、生活质量[生活质量量表(QOL)]、术后并发症发生情况以及随访1年化疗和复发情况。结果 观察组患者手术时间、首次排气时间、首次进食时间、住院时间均明显短于对照组,术中出血量明显少于对照组,差异均有统计学意义(P<0.01)。术后6个月,两组患者KPS、QOL评分均高于本组手术前,且观察组患者KPS、QOL评分均高于对照组,差异均有统计学意义(P<0.05)。观察组患者的术后并发症总发生率为4.00%,低于对照组患者的16.00%,差异有统计学意义(P<0.05)。两组患者的术后化疗率和复发率比较,差异均无统计学意义(P>0.05)。结论腹腔镜淋巴结清扫术治疗卵巢癌的效果显著,可有效缓解患者的疼痛程度,提高生活质量。
Objective To compare the efficacy and safety of laparoscopic lymph node dissection and open lymph node dissection in the treatment of ovarian cancer. Method According to the different treatment methods, 100 patients with ovarian cancer were divided into observation group and control group, 50 cases in each group. The observation group were treated with laparoscopic lymph node dissection, and the control group were treated with open lymph node dissection. The surgery-related indicators, physical status [Karnofsky performance status(KPS) score], quality of life[quality of life scale(QOL)], postoperative complications, and 1-year follow-up chemotherapy and recurrence were compared between the two groups. Result The operation time, the first exhausting time, the first eating time, and the hospitalization time of the observation group were significantly shorter than those of the control group, and the intraoperative blood loss was significantly less than that of the control group, and the differences were statistically significant(P<0.01).Six months after operation, the KPS and QOL scores of the two groups were higher than those before the operation, and the KPS and QOL scores of the observation group were higher than those of the control group, and the differences were statistically significant(P<0.05). The total incidence of postoperative complications in the observation group was 4.00%,which was lower than 16.00% in the control group, and the difference was statistically significant(P<0.05). There were no significant differences in postoperative chemotherapy rate and recurrence rate between the two groups(P>0.05). Conclusion Laparoscopic lymph node dissection has a significant effect in the treatment of ovarian cancer, which can effectively relieve the pain of patients and improve the quality of life.
