经鼻高流量氧疗对慢性阻塞性肺疾病急性加重伴高碳酸血症的疗效探究 被引量:3

Therapeutic effect of nasal high-flow oxygen therapy on acute exacerbation of chronic obstructive pulmonary disease with hypercapnia

摘要 目的探究经鼻高流量氧疗(HFNC)治疗慢性阻塞性肺疾病(COPD)急性加重(AECOPD)伴高碳酸血症患者的临床疗效。方法选取河南省胸科医院2020年1月至2022年1月收治的70例AECOPD伴高碳酸血症患者为研究对象,采用随机数字表分为NPPV组(35例)和HFNC组(35例)。其中NPPV组患者给予NPPV治疗,HFNC组患者给予HFNC治疗。比较两组患者血气指标:血氧饱和度(Sp O)、动脉血二氧化碳分压[p(CO)]、动脉血氧分压[p(O)],肺功能:用力肺活量(FVC)、第1秒用力呼气容积(FEV1)、FEV1/FVC,血液嗜酸性粒细胞(EOS)水平。结果与治疗前比较,两组治疗后Sp O、p(O)、FVC、FEV1、FEV1/FVC水平明显升高(P<0.05),且与NPPV组比较,HFNC组升高更显著(P<0.05);两组治疗后p(CO)水平明显降低(P<0.05),且与NPPV组比较,HFNC组降低更显著(P<0.05)。与治疗前比较,两组治疗后血EOS%、绝对值水平明显降低(P<0.05),且与NPPV组比较,HFNC组降低更显著(P<0.05);HFNC组总有效率91.43%明显高于NPPV组68.57%(P<0.05)。结论HFNC与NPPV用于治疗AECOPD伴高碳酸血症均有一定的治疗效果,但是HFNC能够更加有效的提高患者肺功能,调节血气指标,改善病情。 Objective To investigate the clinical efficacy of high-flow nasal cannula(HFNC)in the treatment of acute exacerbation of chronic obstructive pulmonary disease(COPD)(AECOPD)with hypercapnia.Methods Seventy patients with AECOPD and hypercapnia admitted to Henan Provincial Thoracic Hospital from January 2020 to January 2022 were selected as the research objects.They were randomly divided into NPPV group(35 cases)and HFNC group(35 cases).The patients in the NPPV group were treated with NPPV,and the patients in the HFNC group were treated with HFNC.The blood gas indexes of the two groups were compared:blood oxygen saturation(SpO),arterial partial pressure of carbon dioxide[p(CO)]、,arterial partial pressure of oxygen[p(CO)]、,pulmonary function:forced vital capacity(FVC),forced expiratory volume in the first second(FEV1),FEV1/FVC,and blood eosinophil(EOS)levels.Results Compared with before treatment,the levels of SpO,p(CO),FVC,FEV1,and FEV1/FVC in the two groups were significantly increased after treatment(P<0.05),and compared with the NPPV group,the levels of the HFNC group were increased more significantly(P<0.05).After treatment,p(CO)level in the two groups was significantly decreased(P<0.05),and compared with the NPPV group,the decrease in the HFNC group was more significant(P<0.05).Compared with before treatment,blood EOS%and absolute value levels in the two groups were significantly decreased after treatment(P<0.05),and compared with the NPPV group,the decrease was more significant in the HFNC group(P<0.05).The total effective rate of the HFNC group(91.43%)was significantly higher than that of the NPPV group(68.57%),P<0.05.Conclusion Both HFNC and NPPV have certain therapeutic effects in the treatment of AECOPD with hypercapnia,but HFNC can more effectively improve the lung function of patients,regulate blood gas indexes,and improve the disease.
