摘要
目的探讨胰岛素样生长因子1(insulin-like growth factor 1,IGF-1)单核苷酸多态性与非小细胞肺癌(non-small cell lung cancer,NSCLC)患者疼痛和生活质量的关系。方法回顾性分析2019年9月至2021年12月宁波大学附属人民医院收治的398例NSCLC患者,采用TaqMan探针法对研究对象的IGF-1基因位点(rs6214、rs35767、rs5742612)进行基因分型,采用数字评分法(numerical rating scales,NRS)和8条目健康调查简表(short-form health survey-8,SF-8)评价患者的疼痛和生活质量,采用单因素和多因素Logistic回归分析评价疼痛NRS得分和SF-8得分的影响因素。结果未发现rs6214与疼痛和生活质量的关系;在NSCLC患者中,IGF-1 rs5742612 AA、AG、GG基因型携带者的术后3d疼痛NRS评分分别为(5.49±1.76)、(5.53±1.84)、(4.38±1.92)分,术后7d疼痛NRS评分分别为(4.86±1.82)、(4.88±1.95)、(3.91±2.04)分,差异均有统计学意义(P=0.002、0.019)。生活质量评分方面,rs35767GG、GA、AA基因型携带者之间的机体功能得分[(21.23±24.71)vs(18.03±20.53)vs(28.06±23.74)]与躯体角色得分[(60.09±16.28)vs(62.70±16.55)vs(68.37±19.62)]差异均有统计学意义(P=0.022、0.010);rs5742612AA、AG、GG基因型携带者的生命力得分分别为(41.22±18.25)、(41.51±19.86)、(51.47±22.14)分,差异有统计学意义(P=0.014)。多因素Logistic回归分析显示,rs5742612基因多态性为术后3、7d疼痛NRS评分的独立影响因素(P=0.008、0.010),rs35767基因多态性为躯体角色得分的独立影响因素(P<0.001)。结论IGF-1基因多态性与NSCLC患者的疼痛和生活质量有关,可为更好地实现个体化疼痛和生活质量提供有价值的信息。
Objective To investigate the association of insulin-like growth factor 1(IGF-1)single nucleotide polymorphism with pain and quality of life in patients with non-small cell lung cancer(NSCLC).Methods Three hundred and ninety-eight patients with NSCLC who were treated in the Affiliated People's Hospital of Ningbo University from September 2019 to December 2021 were retrospectively selected as the research subjects.The IGF-1 loci(rs6214,rs35767,rs5742612)were genotyped by the TaqMan probe method,and the pain and life of the patients were evaluated by the Numerical Rating Scales(NRS)and Short-Form Health Survey-8(SF-8).Univariate and multivariate logistic regression analysis was used to evaluate the influencing factors of pain NRS and SF-8 score.Results No relationship between rs6214 and pain and quality of life was found;In NSCLC patients,the postoperative 3d and 7d pain NRS in IGF-1 rs5742612 AA,AG,GG genotype carriers were(5.49±1.76),(5.53±1.84),(4.38±1.92),(4.86±1.82),(4.88±1.95),(3.91±2.04),the differences were statistically significant(P=0.002,0.019).In the quality of life score,the body function scores and body role scores between rs35767GG,GA,and AA genotype carriers were(21.23±24.71)vs(18.03±20.53)vs(28.06±23.74)and(60.09±16.28)vs(62.70±16.55)vs(68.37±19.62),respectively,the differences were statistically significant(P=0.022,0.010);the role-physical scores between rs5742612AA,AG,GG genotype carriers were(41.22±18.25),(41.51±19.86),(51.47±22.14),the differences were statistically significant(P=0.014).The multivariate logistic regression analysis showed that rs5742612 gene polymorphism was an independent influencing factor of postoperative 3d and 7d pain NRS(P=0.008,0.010),and rs35767 gene polymorphism was an independent influencing factor of role-physical score(P<0.001).Conclusion IGF-1 gene polymorphisms are associated with pain and quality of life in patients with NSCLC,which can provide valuable information for better individualized pain and quality of life services.
