摘要
目的对我院门急诊药房处方前置审核的实施效果进行分析和评价,以期对门诊合理用药提供参考。方法选取乐山市中医医院2020年1~6月处方前置审核系统使用前及2021年1~6月处方前置审核系统使用后门诊处方,对比两个时间段处方合理率,并分析审方系统使用后处方问题变化趋势。结果2021年1~6月处方合理率显著高于2020年1~6月处方合理率,系统使用之后,处方合理率由2020年1月的73.30%升至2021年6月的90.95%。“剂量范围问题”与“给药途径问题”呈现下降趋势,与整体不合格率变化趋势一致;但“超适应症问题”与“重复用药问题”数据表现总体平稳,“超多日用量”问题则上涨明显,这与审方系统规则不完善有关。结论处方前置审核系统的使用有效提高了处方质量,优化了就诊流程,促进了医师合理用药,但系统本身还存在规则更新滞后等问题需要解决。
OBJECTIVE To analyze and evaluate the implementation effect of prescription pre audit in outpatient and emergency Pharmacy of our hospital,in order to provide reference for rational drug use in outpatient department.METHODS The outpatient prescriptions of our hospital before the use of the prescription pre audit system from January to June 2020 and after the use of the prescription pre audit system from January to June 2021 were selected,the reasonable rates of prescriptions in the two time periods were compared,and the change trend of prescription problems after the use of the prescription review system was analyzed.RESULTS The reasonable rate of prescription from January to June 2021 was significantly higher than that from January to June 2020.After the system was used,the reasonable rate of prescription increased from 73.30%in January 2020 to 90.95%in June 2021.“Dose range problem”and“administration route problem”showed a downward trend,which was consistent with the change trend of the overall unqualified rate;However,the data of“over indication problem”and“repeated drug use problem”are generally stable,and the problem of“over multi day dosage”has increased significantly,which is related to the imperfect rules of the trial system.CONCLUSION The use of prescription pre audit system effectively improves the prescription quality,optimizes the treatment process and promotes doctors′rational drug use,but the system itself still has some problems to be solved,such as the lag of rule update.
