摘要
目的对比Sturge-Weber综合征(SWS)脑部与面部血管瘤影像表现,结合文献分析其形成机制。方法搜集2011年9月至2021年5月在华中科技大学同济医学院附属协和医院、武汉红十字会医院经临床与影像学检查确诊的7例SWS患者的影像与临床资料,7例全部行脑CT平扫,5例行脑MRI平扫,其中3例行MRI增强扫描、2例行磁敏感加权成像(SWI)检查,对比分析脑部和面部血管瘤CT和MRI影像征象,结合文献分析脑部与面部血管瘤形成与影像学表现差异的机制。结果SWS综合征患者Ⅰ型4例,Ⅱ型1例,Ⅲ型2例,5例颜面血管瘤,影像表现为患侧皮肤增厚、明显强化,2例病灶累及眶周、颌部软组织,伴骨肥大,均无钙化;6例患者脑皮层钙化、强化及脑萎缩,2例SWI检查显示脑皮层、皮层下极低信号。结论SWS脑部与面部血管瘤起源相同,影像表现存在明显差异,CT与MRI能反映脑部与面部病灶的各自特征及不同的发生机制。
Objective To compare the imaging features of cerebral and facial hemangioma in Sturge-Weber syndrome,and analyze its formation mechanism with literature.Methods From September 2011 to May 2021,the images and clinical data of 7 patients with Sturge-Weber syndrome diagnosed by clinical and imaging examinations in Tongji Medical College Affiliated Hospital of Huazhong University of Science and Technology and Wuhan Red Cross Hospital were collected.All 7 patients underwent plain brain CT scan,3 patients underwent MRI enhancement and 2 patients underwent SWI examination in 5 patients.The CT and MRI imaging signs of brain and facial hemangioma were compared and analyzed,and the mechanism of the difference between the formation and imaging manifestations of brain and facial hemangioma was analyzed with literature.Results Sturge-Weber syndrome included 4 cases of typeⅠ,1 case of typeⅡ,2 cases of typeⅢ,and 5 cases of facial hemangioma.The imaging findings showed that the skin of the affected side was thickened and obviously strengthened,and 2 cases involved soft tissues around the orbit and jaw,accompanied by bone hypertrophy,and no calcification.6 cases had calcification,enhancement and brain atrophy in the cerebral cortex,and 2 cases showed extremely low signal intensity in the cerebral cortex and subcortical area by SWI.Conclusion Sturge-Weber syndrome has the same origin as facial hemangioma,and there are obvious differences in imaging manifestations.CT and MRI can reflect the respective characteristics and different pathogenesis of brain and facial lesions.
