摘要
目的:观察肌电生物反馈疗法在痉挛型脑瘫患儿康复治疗中的应用效果。方法:选取2019年1月至2020年12月该院收治的92例痉挛型脑瘫患儿进行前瞻性研究,按照随机数字表法分为对照组和观察组各46例。对照组实施常规康复治疗,观察组在对照组基础上联合肌电生物反馈疗法治疗,比较两组临床疗效、治疗前后运动功能[粗大运动功能评估表(GMFM-88)]评分、日常生活能力量表(ADL)评分,发育商(Gesell小儿神经心理发育量表)评分及血清学指标[谷氨酸(GLU)、γ-氨基丁酸(GABA)、白细胞介素-4(IL-4)、肿瘤坏死因子-α(TNF-α)]水平。结果:观察组治疗总有效率为97.83%,高于对照组的82.61%,差异有统计学意义(P<0.05);治疗后,观察组GMFM-88、ADL评分均高于对照组,差异有统计学意义(P<0.05);观察组适应行为、大运动行为、精细动作行为、个人-社交行为等发育商各项评分均较对照组增高,差异有统计学意义(P<0.05);观察组GLU、TNF-α水平均低于对照组,GABA、IL-4水平均高于对照组,差异有统计学意义(P<0.05)。结论:肌电生物反馈疗法联合常规康复治疗应用于痉挛型脑瘫患儿康复治疗,可提高临床疗效,改善患儿粗大运动功能和血清学指标水平,提高发育商和自理能力评分,其效果优于单纯常规康复治疗。
Objective:To observe application effects of electromyography biofeedback therapy in rehabilitation treatment of children with spastic cerebral palsy.Methods:A prospective study was conducted on 92 children with spastic cerebral palsy who were admitted to this hospital from January 2019 to December 2020.They were divided into control group and observation group according to the random number table method,46 cases in each group.The control group received conventional rehabilitation therapy,while the observation group received electromyography biofeedback therapy on the basis of that of the control group.The clinical efficacy,the motor function scores[gross motor function measure(GMFM-88)],activities of daily living(ADL)score,the developmental quotient(Gesell pediatric neuropsychological development scale)score,and the serological indicator levels[glutamate(GLU),gamma-aminobutyric acid(GABA),interleukin-4(IL-4),tumor cell necrosis factor-α(TNF-α)]were compared between the two groups before and after treatment.Results:The total effective rate of the observation group was 97.83%,which was higher than 82.61%of the control group,and the difference was statistically significant(P<0.05).After the treatment,the GMFM-88 score and the ADL score of the observation group were higher than those of the control group,and the differences were statistically significant(P<0.05).The scores of adaptive behavior,gross motor behavior,fine motor behavior and individual-social behavior in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).The levels of GLU and TNF-αin the observation group were lower than those in the control group,while the levels of GABA and IL-4 in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusions:The electromyography biofeedback therapy combined with routine rehabilitation training in the rehabilitation treatment of the children with spastic cerebral palsy can improve clinical efficacy,improve the gross motor function and the serological indicator levels,and improve the developmental quotient and the self-care ability scores.Moreover,it is superior to single conventional rehabilitation treatment.
