摘要
目的通过系统评价中药外敷治疗哺乳期乳头皲裂患者的临床疗效,从循证医学的角度分析临床效果。方法计算机检索PubMed、EMbase、The Cochrane Library、中国生物医学文献数据库、中国知网、万方数据库、维普数据库中关于中药外敷治疗哺乳期乳头皲裂的随机对照试验,检索时间为建库以来至2021年8月。通过文献筛选、资料提取,用“Cochrane偏倚风险评估工具”和Jadad评分量表对各研究进行质量评价,采用RevMan 5.4软件进行meta分析。结果最终纳入6项随机对照研究,总样本数为644例,其中试验组357例,对照组287例。试验组的临床总有效率高于对照组,差异具有统计学意义(OR=4.18,95%CI:2.43~7.20,P<0.00001);红肿消退率高于对照组,差异有统计学意义(OR=6.14,95%CI:1.32~28.63,P=0.02);疼痛减轻率高于对照组,差异有统计学意义(OR=3.06,95%CI:1.64~5.71,P<0.0004)。结论中药外敷治疗哺乳期乳房皲裂具有优势,安全性高,值得临床推广。
Objective To systematically evaluate the clinical efficacy of external application of traditional Chinese medicine in the treatment of lactating patients with chapped nipples,and analyze the clinical effects from the perspective of evidence-based medicine.Methods Computer search PubMed,EMbase,The Cochrane Library,China Biomedical Literature Database,China National Knowledge Infrastructure,Wanfang Database,VIP Database for randomized controlled trials of external application of traditional Chinese medicine in the treatment of breast-feeding chapped nipples Experiment,the retrieval time is from the establishment of the database to August 2021.Through literature screening and data extraction,the"Cochrane Bias Risk Assessment Tool"and Jadad rating scale were used to evaluate the quality of each study,and RevMan 5.4 software was used for meta-analysis.Results Finally,6 randomized controlled trials(RCTs)were included,with a total of 644 samples,including 357 in the experimental group and 287 in the control group.The clinical total effective rate of the experimental group was higher than that of the control group,and the difference was statistically significant(OR=4.18,95%CI:2.43-7.20,P<0.00001).The redness and swelling regression rate was higher than that of the control group,and the difference was statistically significant(OR=6.14,95%CI:1.32-28.63,P=0.02).The pain reduction rate was higher than that of the control group,and the difference was statistically significant(OR=3.06,95%CI:1.64-5.71,P<0.0004).Conclusion Existing evidence shows that external application of traditional Chinese medicine has advantages and high safety in the treatment of breast chapped breasts during lactation,and is worthy of clinical promotion.
